全程医护一体个体化护理对腋窝路径腔镜甲状腺肿瘤术后患者生活质量的影响

摘　　要：目的分析全程医护一体个体化护理对腋窝路径腔镜甲状腺肿瘤术后患者生活质量的影响。方法选取福建省诏安县医院2022年10月至2023年9月期间收治的152例甲状腺肿瘤患者进行研究,采用随机数字表法将患者分为对照组与试验组,每组76例。对照组围手术期给予常规护理,而试验组给予全程医护一体个体化护理。观察两组干预前后临床评分指标[疾病接受量表(AIS-CHI)、心理弹性量表(CD-RISC)、健康促进生活方式量表(HPLPⅡ)和生活质量量表(SF-36)评分]、术后康复指标(拔管时间、下床活动时间、住院时间以及住院费用)及并发症发生率。结果干预后,试验组AIS-CHI评分、CD-RISC评分、HPLPⅡ评分和SF-36评分高于对照组(P<0.05);试验组恢复时间更短,住院费用更低(P<0.05);并发症总发生率低于对照组(P<0.05)。结论在腋窝路径腔镜甲状腺肿瘤术中,全程医护一体个体化护理不仅能够提高患者疾病接受度、心理弹性,改善健康行为,提高生活质量,也能够加速术后恢复,减轻经济负担,降低并发症风险,提高手术效果,值得推广。Objective To analyze the impact of comprehensive medical and nursing integrated individualized care on the quality of life of patients after thyroid tumor surgery via the axillary approach using endoscopy.Methods A total of 152 patients with thyroid tumors treated at Zhao’an County Hospital were selected from October 2022 to September 2023.Patients were randomly divided into a control group and an trial group,with 76 cases in each group.The control group received routine care during the perioperative period,while the trial group received comprehensive medical and nursing integrated individualized care.Clinical score indicators[Acceptance of Illness Scale(AIS-CHI),Connor-Davidson Resilience Scale(CD-RISC),Health-Promoting Lifestyle ProfileⅡ(HPLPⅡ),and Quality of Life Scale(SF-36)],postoperative recovery indicators(time to tube removal,time to get out of bed,hospital stay,and hospitalization costs),and the incidence of complications were observed before and after intervention in both groups.Results After intervention,the trial group had higher AIS-CHI,CD-RISC,HPLPⅡ,and SF-36 scores than the control group(P<0.05).The trial group had shorter recovery times and lower hospitalization costs(P<0.05),and the overall incidence of complications was lower than that of the control group(P<0.05).Conclusions In axillary approach endoscopic thyroid tumor surgery,comprehensive medical and nursing integrated individualized care not only improves patients’acceptance of illness,psychological resilience,and health behaviors,but also enhances the quality of life,accelerates postoperative recovery,reduces economic burden,lowers the risk of complications,and improves surgical outcomes,making it worthy of promotion.
