摘 要:目的研究和分析结直肠癌患者营养状态与奥沙利铂化疗致周围神经毒性(OIPN)的关系。方法选取福建省肿瘤医院收治的72例结直肠癌患者作为研究对象,围化疗时间2023年1月至2024年3月,其中47例发生OIPN,设为观察组;25例未发生OIPN,设为对照组。两组均行含奥沙利铂方案化疗,并对营养指标和饮食情况进行采集。对两组并发症发生情况、营养风险筛查2002(NRS2002)评分、血红蛋白水平、白蛋白水平进行单因素分析及多因素Logistic回归分析。结果两组并发症发生情况、NRS 2002评分、血红蛋白水平、白蛋白水平比较,差异有统计学意义(P<0.05)。并发症、白蛋白水平偏低是OIPN的独立危险因素(P<0.05)。结论白蛋白水平是评价营养状态的重要指标,白蛋白水平偏低是OIPN的独立危险因素,此外OIPN的发生也与并发症密切相关,患者的饮食观念与饮食习惯仍存在误区,影响营养状态,应加强对患者的营养相关知识宣教,改善营养状态。Objective To investigate and analyze the relationship between nutritional status and peripheral neurotoxicity(OIPN)induced by oxaliplatin chemotherapy in patients with colorectal cancer.Methods A total of 72 colorectal cancer patients admitted to our hospital were selected as the study subjects,with chemotherapy time from January 2023 to March 2024.Among them,47 cases developed OIPN were set as the observation group,25 cases did not develop OIPN were set as the control group.Both groups received chemotherapy with an oxaliplatincontaining regimen and nutritional indicators were collected.Univariate analysis and multivariate logistic regression analysis were conducted on the occurrence of complications,NRS200 score,hemoglobin level,albumin level in two groups.Results The incidence of complications in two groups,NRS 2002 scores,hemoglobin levels,and albumin levels were compared,with statistically significant differences(P<0.05).Complications and low albumin levels are independent risk factors for OIPN(P<0.05).Conclusions Albumin level is an important indicator for evaluating nutritional status,and low albumin level is an independent risk factor for OIPN.In addition,the occurrence of OIPN is closely related to complications.The dietary concepts and habits of patients still have misconceptions,which affect their nutritional status.There should be an increased focus on educating patients about nutrition-related knowledge to improve their nutritional status.
