摘 要:目的:探究血清雌二醇(E_(2))、淀粉样蛋白A(SAA)及全身免疫炎症指数(SII)对早期乳腺癌患者术后预后不良的预测价值。方法:回顾性选取2021年5月—2022年5月某院行乳腺癌改良根治术的早期乳腺癌患者92例为观察组,同期健康体检女性90例为对照组,比较2组血清E_(2)、SAA、SII水平。根据观察组术后2年内有无复发或转移分为预后不良组(25例)和预后良好组(67例),采用单因素、多因素Logistic回归分析早期乳腺癌患者术后预后不良的影响因素;绘制ROC曲线分析血清E_(2)、SAA、SII水平对乳腺癌患者术后预后不良的预测价值。结果:观察组血清E_(2)、SAA、SII水平均高于对照组,差异有统计学意义(P<0.05)。不同预后患者年龄、BMI、高血压、糖尿病、ASA分级及病理学类型情况比较,差异无统计学意义(P>0.05);预后不良组肿瘤直径>2 cm、临床分期为Ⅲ期患者比例及血清E_(2)、SAA、SII水平高于预后良好组,差异有统计学意义(P<0.05)。Logistic回归分析显示,E_(2)、SAA、SII均为乳腺癌患者预后不良的影响因素(P<0.05)。ROC分析显示,血清E_(2)、SAA、SII水平预测早期乳腺癌患者术后短期预后不良的AUC分别为0.853、0.863、0.833;三者联合诊断的AUC为0.947,敏感度为90.00%,特异度为98.51%。结论:早期乳腺癌术后预后不良患者E_(2)、SAA、SII水平均异常升高,三者联合在诊断乳腺癌患者预后中具有较好的效能。Objective:To investigate the predictive analyses of serum estradiol(E_(2)),amyloid A(SAA)and systemic immunoinflammatory index(SII)for poor postoperative prognosis in patients with early breast cancer.Methods:From May 2021 to May 2022,92 patients with early breast cancer undergoing modified radical surgery for breast cancer in a hospital were selected as an observation group,and 90 healthy check-up women at the same time were selected as a control group.Serum E_(2),SAA and SII levels of the two groups were compared.The observation group was divided into a poor prognosis group(25 cases)and a good prognosis group(67 cases)according to whether there was recurrence or metastasis in 2 years after surgery.Single factor and multi factor logistic regression were used to analyze the factors influencing the poor prognosis of early breast cancer patients after surgery.The ROC curve was drawn to analyze the predictive value of serum E_(2),SAA and SII levels for poor prognosis of breast cancer patients after surgery.Results:Serum levels of E_(2),SAA,and SII in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in age,BMI,and pathological type among patients with different prognoses(P>0.05).The proportions of patients with tumors>2 cm and clinical stage III,as well as serum E_(2),SAA,and SII levels,in the poor prognosis group,were higher than those in the good prognosis group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that E_(2),SAA and SII were all factors influencing the poor prognosis of breast cancer patients(P<0.05).ROC analysis showed that the AUC predicted by serum E_(2),SAA and SII levels for poor short-term prognosis in early breast cancer patients were 0.853,0.863 and 0.833,respectively.The AUC for the combined diagnosis of E_(2),SAA,and SII is 0.947,with a sensitivity of 90.00%and a specificity of 98.51%.Conclusion:The levels of E_(2),SAA and
