中医治疗颈动脉粥样硬化的研究概况

摘　　要：颈动脉粥样硬化是临床较为常见的慢性疾病,是心脑血管疾病的主要病理因素,阻止和逆转其病程具有重要意义。目前,西医临床治疗本病以降脂、稳定斑块、抗血小板聚集等为主,药物通常会引起明显的不良反应,防治效果往往不理想。中医药、特色疗法等防治颈动脉粥样硬化具有良好的实践基础和应用前景。中医在临床治疗上具有全方位和多靶点的作用特征,能够作用于动脉粥样硬化发生发展的各个阶段与环节,具有疗效好、不良反应小的优点。中医学认为,颈动脉粥样硬化是本虚标实之证,基本病机为“虚、瘀、痰、火”四者相互搏结,以致血络不通,脉道不利。治疗方法主要有活血化瘀、补虚泻实、祛痰化浊等,可有效调节血脂,改善血管内皮功能,减轻炎症反应,延缓斑块进程,甚至逆转斑块。中医针刺、艾灸、穴位贴敷等特色疗法是临床上重要的辅助治疗方式,可有效延缓斑块进程,缓解患者的临床症状,提高生活质量。现阶段仍存在一些问题,如关于中医预防颈动脉粥样硬化机制的研究较少,今后应扩大样本量进一步研究。近年来,中医药在我国心血管疾病的防治中发挥着日益重要的作用,很多专家学者对中医药防治颈动脉粥样硬化做了大量研究。本研究查阅近15年文献,对中医药、中医特色疗法防治颈动脉粥样硬化的研究进展进行综述,以期为临床防治颈动脉粥样硬化拓展思路。Carotid atherosclerosis(CAS)is a common chronic disease,and the main pathological factor of cardiovascular and Ccerebrovascular diseases.It is of great significance to prevent and reverse the course of CAS.At present,Western medicine mainly treats this disease by lowering blood lipid,stabilizing plaque and resisting platelet aggregation in clinic,which usually causes obvious adverse reactions.The prevention and treatment effect is often not desired.Traditional Chinese medicine has good practice foundation and application prospect in preventing and treating CAS.Traditional Chinese medicine has omni-directional and multi-target action characteristics in clinical treatment,which can act on all stages and links of atherosclerosis,and has the advantages of good curative effects and adverse reactions.In traditional Chinese medicine,CAS is considered to be the syndrome of deficiency and excess.The basic pathogenesis of CAS is‘deficiency,blood stasis,phlegm and fire’,which leads to the obstruction of blood collaterals.The unfavorable treatments mainly include promoting blood circulation and removing blood stasis,tonifying deficiency and purging excess,and eliminating phlegm and eliminating turbidity,which can effectively regulate blood lipid,improve vascular endothelial function,reduce inflammatory reaction,delay plaque evolvement and even reverse plaque.Traditional Chinese medicine moxibustion,acupoint application and other special therapies are important clinical adjuvant treatments,which can effectively delay plaque evolvement,relieve clinical symptoms and improve patients’lives.At present,there are still some problems to be solved.For example,there are few studies on the mechanism of preventing CAS in traditional Chinese medicine.In the future,we should expand the sample size and further study.In recent years,traditional Chinese medicine plays an increasingly important role in the prevention and treatment of cardiovascular diseases in China.Many experts and scholars have done a lot of research on the prevention
