摘  要:目的总结卒中后认知障碍患者运动干预的最佳证据。方法按照“6S”循证资源金字塔证据模型,计算机检索澳大利亚乔安娜布里格斯研究所(JBI)循证卫生保健中心、苏格兰院际指南网、UpToDate、美国国立指南文库、BMJ Best Practice、加拿大安大略省注册护士协会网站、加拿大医学会临床实践指南数据库、英国国家卫生与临床优化研究所网站、国际指南协作网、欧洲神经病学学会官网、CochraneLibrary、Embase、PubMed、WebofScience、CINAHL、医脉通、中国知网、万方数据知识服务平台、中国生物医学文献数据库、维普网中关于卒中后认知障碍患者运动干预的文献,检索时间为建库至2024年8月,由两名研究人员进行文献筛选及内容提取、质量评价、证据提取与证据等级评价。结果共纳入21篇文献,其中系统评价/Meta分析11篇、指南4篇、专家共识2篇、证据总结2篇、随机对照试验2篇。最终形成了卒中后认知障碍患者运动干预的最佳证据,其包括运动干预必要性、运动干预前评估、运动干预治疗原则、运动干预处方(方式、强度、频率、注意事项)、运动干预效果评价5个方面共31条证据。结论本研究汇总了卒中后认知障碍患者运动干预的最佳证据,其包括运动干预必要性、运动干预前评估、运动干预治疗原则、运动干预处方(方式、强度、频率、注意事项)、运动干预效果评价5个方面共31条证据,临床医护人员应根据患者具体情况个性化实施运动干预,以改善患者认知功能,促进其早日康复。Objective To summarize the best evidence of exercise intervention in patients with post-stroke cognitive impairment.Methods According to the"6S"evidence based resource pyramid model,the articles related to exercise intervention in patients with post-stroke cognitive impairment was retrieved from Joanna Briggs Institute(JBI)Evidence Based Health Care Centre,Scottish Intercollegiate Guidelines Network,UpToDate,National Guidelines Clearinghouse,BMJ Best Practice,Registered Nurses'Association of Ontario,Canadian Medical Association Clinical Practice Gudelines Database,National Institute for Health and Care Excellence,Guidelines International Network,European Academy of Neurology website,Cochrane Library,Embase,PubMed,Web of Science,CINAHL,Medlive,CNKI,Wanfang Data,Chinese Biomedical Literature Database and VIP.The retrieval time was from the establishment of the database to August 2024.Two researchers screened the literature,extracted the content,assessed the quality,and conducted the evidence extraction and evaluation.Results A total of 21 literature were included,including 11 systematic reviews/meta-analyses,4 guidelines,2 expert consensus,2 evidence summaries,and 2 randomized controlled trials.The best evidence of exercise intervention for patients with post-stroke cognitive impairment was finally formed,including 31 evidence in 5 aspects:exercise intervention necessity,assessment before exercise interverction,treatment principle of exercise intervention,prescription of exercise intervention(modality,intensity,frequency,and precautions),and effect evaluation of exercise intervention.Conclusion This study summarizes the best evidence of exercise intervention in patients with post-stroke cognitive impairment,including 31 evidence in 5 aspects:exercise intervention necessity,assessment before exercise interverction,treatment principle of exercise intervention,prescription of exercise intervention(modality,intensity,frequency,and precautions),and effect evaluation of exercise intervention.Medical workers should implem
