血府逐瘀汤加减联合针刺治疗缺血性视神经病变临床观察

摘　　要：目的:探究血府逐瘀汤加减联合针刺治疗缺血性视神经病变的临床效果。方法:选取邯郸市第三医院2021年8月~2023年8月期间收治的106例缺血性视神经病变患者作为研究对象,随机分为对照组和研究组,各53例,对照组采用常规西医治疗,研究组在此基础上给予血府逐瘀汤加减联合针刺治疗。比较两组临床效果。结果:研究组治疗2周临床总有效率为94.34%,高于对照组79.25%(P<0.05);治疗后两组中医症候主证及次证积分均较治疗前下降,且研究组较对照组更低,差异具有统计学意义(P<0.05);与治疗前比较,两组治疗后患眼动脉阻力指数(RI)、视野平均缺损下降,收缩期峰值血流速度(PSV)、最佳矫正视力、视野平均光敏感度升高,与对照组比较,研究组上述指标变化幅度更大,差异具有统计学意义(P<0.05)。结论:血府逐瘀汤加减联合针刺治疗缺血性神经病变可以有效改善患者中医症状,改善眼动脉血流动力学参数,促进视力及视野缺损的恢复,有助于提升西医治疗效果。Objective:To explore the clinical effect of Xuefu Zhuyu Decoction combined with acupuncture on ischemic optic neuropathy.Methods:106patients with ischemic optic neuropathy admitted to the Third Hospital of Handan City from August 2021to August 2023were selected as the study objects and randomly divided into control group(53cases)and study group(53cases).The control group was treated with conventional Western medicine,and the study group was treated with Xuefu Zhuyu Decoction combined with acupuncture on this basis.The clinical effect of the two groups was compared.Results:Compared with the control group,the total effective rate of the study group was higher significantly(94.34%vs 79.25%,P<0.05).After treatment,the scores of the main syn-drome and secondary syndrome of TCM in both groups were lower than before treatment,and compared with the control group,the score in the study group was lower significantly(P<0.05).Compared with before treatment,the affected ocular arterial resistance index(RI),average visual field defect decreased,peak systolic blood flow velocity(PSV),best corrected visual acuity and average visual field light sensitivity increased in both groups after treatment.Compared with the control group,the above indexes changed more significantly in the study group(P<0.05).Conclusion:Xuefu Zhuyu Decoction combined with acupuncture in the treatment of ischemic neuropathy can effectively improve the symptoms of traditional Chinese medicine,improve the parameters of blood flow of the eye artery,promote the recovery of visual acuity and visual field defects,and help to improve the effect of western medicine.
