摘 要:目的探讨急性脑出血(AICH)患者外周血微小RNA(miRNA)-124-3p、miR-27a-3p的表达与病情严重程度和预后的关系。方法选取2020-03—2022-11南京市江宁医院收治的110例AICH患者为AICH组,另选取同期55名体检健康志愿者为对照组,根据美国国立卫生研究院卒中量表(NIHSS)将AICH患者分为轻度组(37例)、中度组(44例)、重度组(29例),根据改良Rankin量表评分分为预后不良组(33例)、预后良好组(77例)。采用qPCR检测外周血miR-124-3p、miR-27a-3p表达。AICH患者NIHSS评分与外周血miR-124-3p、miR-27a-3p表达的相关性采用Spearman相关性分析,AICH患者预后不良的影响因素采用多因素Logistic回归分析,外周血miR-124-3p、miR-27a-3p表达对AICH患者预后不良的预测价值采用ROC曲线分析。结果AICH组外周血miR-124-3p、miR-27a-3p表达水平低于对照组(P<0.05)。轻度组、中度组、重度组外周血miR-124-3p、miR-27a-3p表达水平依次降低(P<0.05)。AICH患者外周血miR-124-3p、miR-27a-3p表达水平与NIHSS评分呈负相关(P<0.05)。血肿体积≥10.00 mL、NIHSS评分≥11.50分、去骨瓣减压术为AICH患者预后不良的独立危险因素,miR-124-3p、miR-27a-3p升高为独立保护因素(P<0.05)。外周血miR-124-3p联合miR-27a-3p预测AICH患者预后不良的曲线下面积大于miR-124-3p、miR-27a-3p单独检测(P<0.05)。结论AICH患者外周血miR-124-3p、miR-27a-3p的表达与病情严重程度和预后有关,可能成为AICH患者预后不良的辅助预测指标。Objective To investigate the relationship between the expressions of peripheral blood microRNA(miRNA)-124-3p and miR-27a-3p in patients with acute intracerebral hemorrhage(AICH)and the severity and prognosis of the disease.Methods A total of 110 patients with AICH admitted to Nanjing Jiangning Hospital from March 2020 to November 2022 were selected as the AICH group,and 55 healthy volunteers with medical checkups during the same period were selected as the control group,and the patients with AICH were classified into the mild group(37 cases),moderate group(44 cases),severe group(29 cases),or poor prognosis group(33 cases)and good prognosis group(77 cases)according to the scores of National Institutes of Health stroke scale(NIHSS)and modified Rankin scale.Peripheral blood miR-124-3p and miR-27a-3p expressions were detected by qPCR.The correlation between NIHSS score and peripheral blood miR-124-3p and miR-27a-3p expressions in AICH patients was analyzed by Spearman correlation analysis,the influencing factors for poor prognosis in AICH patients were analyzed by multifactorial Logistic regression analysis,and the expressions of miR-124-3p and miR-27a-3p in peripheral blood in predicting poor prognosis of patients with AICH was analyzed by ROC curve.Results The expressions of miR-124-3p and miR-27a-3p in peripheral blood of the AICH group were lower than those of the control group(P<0.05).The expressions of miR-124-3p and miR-27a-3p in peripheral blood were reduced in the mild,moderate and severe groups(P<0.05).Peripheral blood miR-124-3p and miR-27a-3p expressions in patients with AICH were negatively correlated with NIHSS scores(P<0.05).Hematoma volume≥10.00 mL,NIHSS score≥11.50,and debridement decompression were independent risk factors for poor prognosis in patients with AICH,and elevated miR-124-3p and miR-27a-3p were independent protective factors(P<0.05).The area under the curve of peripheral blood miR-124-3p combined with miR-27a-3p predicting poor prognosis in patients with AICH more than miR-124-3p and
