外展神经鞘瘤致听力障碍、面部麻木1例并文献复习

摘　　要：目的 分析外展神经鞘瘤致听力障碍、面部麻木的诊疗过程及方法。方法 回顾性分析2020年1月湘雅常德医院收治的1例外展神经鞘瘤患者的临床资料,并结合相关文献进行复习。结果 患者采用枕下乙状窦后入路全切肿瘤术后,病理检查结果为神经鞘瘤,患者右侧耳鸣、面部麻木感消失,听力缓解,右侧眼球外展受限明显,面神经功能3级。术后1年半随访患者面神经功能恢复至术前,右侧眼球活动依然外展受限。术后随访未见肿瘤复发。结论 外展神经鞘瘤是一种少见的起源于单纯支配运动的神经鞘瘤,作为不常见肿瘤在术前很容易被忽略,首选治疗方案为手术完整切除。Objective To analyze the diagnosis and treatment process and methods of hearing impairment and facial numbness caused by external nerve sheath tumors.Methods The clinical data of a patient with abductor nerve sheath tumor admitted to Xiangya Changde Hospital in January 2020 were analyzed retrospectively,and the relevant literature were reviewed.Results The patient underwent a total resection of the tumor through the retrosigmoid sinus approach under the pillow,and the pathological examination showed a schwannoma.The patient s right tinnitus and facial numbness disappeared,hearing was relieved,and the right eyeball abduction was significantly restricted.The facial nerve function was grade 3.One and a half years after surgery,the patient s facial nerve function recovered to preoperative levels,but the right eye movement was still limited in abduction.No tumor recurrence was observed during postoperative follow-up.Conclusions Abducens schwannoma is a rare type of solely motor nerve sheath tumor,easily to be overlooked before surgery,and the preferred treatment option is complete surgical resection.
