摘　　要：目的探究结直肠癌患者血清巨噬细胞炎性蛋白-3α(MIP-3α)、白细胞介素-37(IL-37)、CC类趋化因子配体5(CCL5)水平并分析其临床意义。方法前瞻性选择辽宁省肿瘤医院2018年1月至2020年12月收治的结直肠癌患者158例作为癌症组,并根据随访情况分为多发组(41例)和无复发组(117例)。另选择同时期在辽宁省肿瘤医院健康查体的健康志愿者158例作为对照组;利用酶联免疫吸附试验(ELISA)检测血清MIP-3α、IL-37、CCL5水平;采用Pearson相关分析癌症组血清中MIP-3α、IL-37、CCL5水平的相关性;采用多因素Logistic回归分析影响结直肠癌患者术后复发的因素;采用受试者工作特征(ROC)曲线分析血清MIP-3α、IL-37、CCL5联合对结直肠癌患者术后复发的预测价值。结果癌症组血清MIP-3α、CCL5水平明显高于对照组,IL-37水平明显低于对照组(P<0.05);与无复发组相比,复发组TNM分期为Ⅲ期的患者比例较高(P<0.05),且复发组血清MIP-3α、CCL5水平明显升高,IL-37水平明显降低(P<0.05);癌症组血清MIP-3α与IL-37水平呈负相关(r=-0.540,P<0.05),与CCL5水平呈正相关(r=0.623,P<0.05),IL-37与CCL5水平呈负相关(r=-0.551,P<0.05);TNM分期为Ⅲ期、MIP-3α水平升高、CCL5水平升高是影响结直肠癌患者术后复发的危险因素(P<0.05),IL-37水平升高是影响结直肠癌患者术后复发的保护因素(P<0.05);血清MIP-3α、IL-37、CCL5联合预测结直肠癌患者术后复发的曲线下面积(AUC)为0.889(95%CI:0.830~0.948),MIP-3α、IL-37、CCL5单独预测结直肠癌患者术后复发的AUC分别为0.780(95%CI:0.692~0.868)、0.755(95%CI:0.673~0.838)、0.803(95%CI:0.730~0.876),三者联合预测的AUC较MIP-3α(Z=2.015,P=0.022),IL-37(Z=2.596,P=0.005),CCL5(Z=1.805,P=0.036)单独预测的AUC更大。MIP-3α高表达患者无复发生存率(55/86,63.95%)低于MIP-3α低表达患者(62/72,86.11%;Log-Rankχ^(2)=11.709,P<0.05);IL-37高表达无复发生存率(86.62%,77/89)高于IL-37低表达患者(57.9Objective To analyze the levels and clinical significance of macrophage inflammatory protein-3α(MIP-3α),interleukin-37(IL-37)and CC chemokine ligand 5(CCL5)in serum macrophages of patients with colorectal cancer.Methods A total of 158 patients with colorectal cancer admitted to Liaoning Cancer Hospital from January 2018 to December 2020 were prospectively selected as the cancer group,and were divided into the recurrence group(41 cases)and the non recurrence group(117 cases)according to the the follow-up situation.158 healthy volunteers who underwent physical examination in Liaoning Cancer Hospital during the same period were selected as control group.Enzyme linked immunosorbent assay(ELISA)was applied to detect serum levels of MIP-3α,IL-37 and CCL5.Pearson correlation was applied to analyze the correlation between serum levels of MIP-3α,IL-3 and CCL5 in cancer group.Multivariate Logistic regression was applied to analyze the factors affecting postoperative recurrence in colorectal cancer patients.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum MIP-3α,IL-37 and CCL5 in combination for postoperative recurrence in colorectal cancer patients.Results The serum levels of MIP-3αand CCL5 in the cancer group were obviously higher than those in the control group,while the level of IL-37 was obviously lower than that in the control group(P<0.05).Compared with the non recurrence group,the proportion of patients with TNM stageⅢin the recurrence group was higher(P<0.05),and the serum levels of MIP-3αand CCL5 in the recurrence group were obviously increased,while the level of IL-37 was obviously reduced(P<0.05).Serum MIP-3αwas negatively correlated with IL-37 level(r=-0.540,P<0.05)and was positively correlated with CCL5 level(r=0.623,P<0.05),and IL-37 was negatively correlated with CCL5 level(r=-0.551,P<0.05).TNM stageⅢ,increased level of MIP-3αand increased level of CCL5 were risk factors for postoperative recurrence in colorectal cancer patients(P<0.05),while incre
