摘　　要：目的 探讨前列腺癌在超声声像图的显著特征,分析经直肠前列腺癌超声图像分型及其与国际泌尿病理学会前列腺癌分级系统(ISUP)的相关性。方法 收集本院行直肠超声引导下经会阴前列腺系统穿刺术患者,对比前列腺癌和前列腺良性病变的超声声像图特点,分析不同类型的前列腺癌声像图与ISUP分级是否存在相关性。结果 在临床及超声资料中,前列腺良、恶性病变组间差异具有统计学意义的指标及特征为年龄、总前列腺特异性抗原、游离前列腺特异性抗原、前列腺的被膜完整性、内外腺分界以及病变部位、回声、形态、血流阻力指数、血流收缩峰值速度。根据经直肠超声声像图特征对前列腺癌进行分型,最多见的类型为结节型(40.6%),其次为弥漫浸润型(33.3%)、弥漫浸润伴结节型(18.8%)和隐匿型(7.2%)。四种前列腺癌超声分型和ISUP分级之间存在显著性关联(P<0.05)。结论 经直肠超声前列腺癌声像图分型与ISUP分级存在显著相关性。Objective To investigate the significant features of prostate cancer on ultrasound images,and analyze the ultrasonographic classification of prostate cancer images via transrectal ultrasound and its correlation with the International Society of Urological Pathology(ISUP) grading system.Methods Patients who underwent transperineal prostate biopsy under the guidance of transrectal ultrasound in our hospital were collected.The ultrasound image characteristics between prostate cancer and benign prostate lesions were compared,and the correlation between the ultrasound image types of different prostate cancers and the ISUP grading was analyzed.Results In the ultrasound data,the characteristics with statistically significant differences between benign and malignant prostate lesions were age,free prostate-specific antigen,total prostate-specific antigen,the integrity of the prostate capsule,the boundary between the inner and outer glands,and the lesion location,echo,morphology,blood flow resistive index,and systolic peak velocity.According to the characteristics of transrectal ultrasound images,prostate cancer was classified into nodular type(40.6%),diffuse infiltrative type(33.3%),diffuse infiltrative with nodular type(18.8%),and occult type(7.2%).There was a statistically significant correlation between the four types of prostate cancer ultrasound classification and the ISUP grading(P<0.05).Conclusions There is a significant correlation between the classification of prostate cancer ultrasound images via transrectal ultrasound and the ISUP grading.
