摘 要:目的探讨显微镜下蛋壳模拟训练在耳鼻咽喉科住院医师耳显微外科培训中的应用价值。方法选取自2023年8月开始在浙江大学医学院附属第二医院耳鼻咽喉科进行第三年规范化培训的7名医生作为研究对象,进行三轮显微镜下蛋壳模拟训练,分别记录去壳时间及成功率、蛋壳雕刻时间及成功率。同时记录每轮模拟训练后临床带教老师对学员操作评价满意度评分。结果学员第二次培训和第三次培训的去壳成功率和雕刻成功率均高于第一次培训,且第三次培训的去壳成功率和雕刻成功率均高于第二次培训,差异均有统计学意义(t分别=-6.43、-13.19、-7.43、-4.18、-9.31、-2.83,P均<0.05);学员第三次培训去壳时间短于第一次培训,第二次和第三次培训雕刻时间均短于第一次,差异均有统计学意义(t分别=2.57、2.50、3.11,P均<0.05)。带教老师对参与培训的医生第二次和第三次培训的操作评分均高于第一次培训,差异均有统计学意义(t分别=-8.91、-19.72,P均<0.05)。结论显微镜下蛋壳模拟训练是提高耳鼻咽喉科住院医师耳显微外科操作技能的有效培训模式。Objective To investigate the application value of eggshell simulation training under a microscope in microscopic ear surgery training for otorhinolaryngology residents.Methods Seven residents in the third year of standardized training at the Department of Otolaryngology,the Second Affiliated Hospital of Zhejiang University School of Medicine since August 2023 were selected as study subjects to receive three rounds of microscopic eggshell simulation training,and the operation time and success rate of eggshell removal and carving were recorded respectively.The satisfaction questionnaire scores of the clinical teachers on the practical skills of the residents after each round of simulation training were also recorded.Results The success rates of eggshell removal and carving in the second and third training were higher than those in the first training,and the success rate of shell removal and carving in the third training was higher than that in the second training,and the differences were statistically significant(t=-6.43,-13.19,-7.43,-4.18,-9.31,-2.83,P<0.05).The time of eggshell removal in the third training was shorter than that in the first training,and the carving time of the second and third training were shorter than that of the first,the differences were statistically significant(t=2.57,2.50,3.11,P<0.05).The total score of the instructor's evaluation of the trainee's operation in the second and third training was higher than that in the first training,and the difference was statistically significant(t=-8.91,-19.72,P<0.05).Conclusion Microscopic eggshell simulation training is an effective training mode to improve the microscopic ear surgery operation skills of otolaryngology residents.
