摘　　要：目的探讨脂蛋白相关磷脂酶A2(Lp-PLA2)和可溶性凝集素样氧化低密度脂蛋白受体-1(sLOX-1)在急性脑梗死(ACI)中的应用。方法选取2021年7月至2023年1月在我院就诊的181例ACI患者为ACI组,同期接受一般临床检查的154名健康人作为对照组。比较两组Lp-PLA2和sLOX-1水平;采用美国国立卫生院卒中量表(NIHSS)评估ACI组入院后的神经功能缺损程度,分析Lp-PLA2和sLOX-1与NIHSS的关系。利用ROC曲线分析单独及联合应用Lp-PLA2和sLOX-1诊断ACI的灵敏度和特异度。结果ACI组的Lp-PLA2、sLOX-1水平高于对照组,差异具有统计学意义(P<0.001)。Lp-PLA2、sLOX-1、Lp-PLA2联合sLOX-1诊断ACI的AUC分别为0.704、0.774、0.787。Lp-PLA2、sLOX-1水平与NIHSS评分呈正相关(P<0.05)。结论Lp-PLA2和sLOX-1水平与ACI的发生及ACI患者神经功能损伤程度显著相关,对ACI具有一定的诊断价值。Objective To explore the clinical application of lipoprotein-associated phospholipase A2(Lp-PLA2)and soluble lection-like oxidized low-density lipoprotein receptor-1(sLOX-1)in acute cerebral infarction(ACI).Methods A total of 181 patients with ACI who were admitted to our hospital from July 2021 to January 2023 were selected as ACI group,and 154 healthy people who received general clinical examination during the same period were selected as control group.The levels of Lp-PLA2 and sLOX-1 of the two groups were compared.The National Institutes of Health Stroke Scale(NIHSS)was used to evaluate the degree of neurological deficits in the ACI group after admission,and the relationship between Lp-PLA2 and sLOX-1 and NIHSS was analyzed.The sensitivity and specificity of Lp-PLA2 and sLOX-1 alone and in combination in the diagnosis of ACI were analyzed by ROC curve.Results The levels of Lp-PLA2 and sLOX-1 in the ACI group were higher than those in the control group,and the differences were statistically significant(P<0.001).The AUC of Lp-PLA2,sLOX-1 and Lp-PLA2 combined with sLOX-1 in the diagnosis of ACI were 0.704,0.774 and 0.787,respectively.The levels of Lp-PLA2 and sLOX-1 were positively correlated with NIHSS score(P<0.05).Conclusion The levels of Lp-PLA2 and sLOX-1 are significantly correlated with the occurrence of ACI and the degree of neurological impairment in ACI patients,and have certain diagnostic value for ACI.
