超声乳化术联合DEX-I治疗白内障合并糖尿病黄斑水肿的效果

摘　　要：目的:探讨超声乳化术(Phaco)联合地塞米松玻璃体内植入剂(DEX-I)治疗白内障合并糖尿病黄斑水肿(DME)的效果。方法:回顾性分析山东省鲁南眼科医院2022年7月—2023年7月就诊的149例白内障合并DME患者,按照治疗方法的不同,将其分为行伴有DEX-I的超声乳化术(Phaco-Dex)组(n=75)和单纯超声乳化术(Phaco)组(n=74)。对比两组患者临床效果、最佳矫正视力(BCVA)、黄斑中心凹视网膜厚度、黄斑体积(MV)及并发症。结果:Phaco-Dex组总有效率为82.67%,高于Phaco组的67.57%,差异有统计学意义(P<0.05);术后3个月,Phaco-Dex组BCVA优于Phaco组,差异有统计学意义(P<0.05);术后3个月,Phaco-Dex组黄斑中心凹视网膜厚度、MV均低于Phaco组,差异均有统计学意义(P<0.05);术后3个月,Phaco-Dex组并发症发生率低于Phaco组,差异有统计学意义(P<0.05)。结论:相较于单独使用Phaco治疗的患者,Phaco联合DEX-I治疗能提高治疗效果,可有效改善患者视力,减少黄斑中心凹视网膜厚度,降低MV和并发症发生率。Objective:To explore the effect of phacoemulsification(Phaco)combined with Dexamethasone Intravitreal Implant(DEX-I)in the treatment of cataract complicated with diabetic macular edema(DME).Method:A total of 149 cases of cataract complicated with DME in Shandong Lunan Eye Hospital from July 2022 to July 2023 were retrospectively analyzed.According to different treatment methods,they were divided into phacoemulsification with DEX-I(Phaco-Dex)group(n=75)and phacoemulsification alone(Phaco)group(n=74).The clinical effect,best corrected visual acuity(BCVA),macular foveal retinal thickness,macular volume(MV)and complications.Result:The total effective rate of Phaco-Dex group was 82.67%,which was higher than 67.57%of Phaco group,the difference was statistically significant(P<0.05).Three months after operation,the BCVA of Phaco-Dex group was better than that of Phaco group,the difference was statistically significant(P<0.05).Three months after operation,the macular foveal retinal thickness and MV in the Phaco-Dex group were significantly lower than those in the Phaco group,the differences were statistically significant(P<0.05).Three months after operation,the incidence of complications in Phaco-Dex group was lower than that in Phaco group,the difference was statistically significant(P<0.05).Conclusion:Compared with patients treated with Phaco alone,Phaco combined with DEX-I can improve the treatment efficiency,improve the vision of patients,improve the morphology of macular area and reduce the incidence of complications.
