摘 要:目的:分析微卫星不稳定性对子宫内膜癌(EC)发展和预后的预测效果。方法:选取2018年1月—2024年6月在玉林市红十字会医院妇科接受手术治疗的EC患者300例为研究对象。术后均接受免疫组织化学法进行检测,分析患者微卫星稳定状态,将其分为微卫星稳定型(MSS)组、低度微卫星不稳定型(MSI-L)组、高度微卫星不稳定型(MSI-H)组。比较三组年龄、病理情况及预后。结果:300例EC患者中,MSS患者218例(72.67%),MSI-L患者23例(7.67%),MSI-H患者59例(19.67%)。三组FIGO分期、糖类抗原125水平、淋巴脉管侵犯情况、淋巴转移情况、P53突变情况比较,差异有统计学意义(P<0.05);三组年龄、分化程度比较,差异无统计学意义(P>0.05)。MSI-H组患者预后不良率高于MSS组,差异有统计学意义(P<0.001)。MSI-H组与MSI-L组预后不良率比较,差异无统计学意义(P=0.303)。结论:在EC发展的过程中,MMR蛋白缺失可能与疾病的FIGO分期、淋巴脉管侵犯、淋巴结转移、糖类抗原125表达情况相关,且MSI-H也可能是EC预后不良的重要影响因素之一。Objective:To analyze the prediction effect of microsatellite instability on the development and prognosis of endometrial carcinoma(EC).Methods:A total of three hundred EC patients who received surgical treatment in the gynecology department of Yulin Red Cross Hospital from January 2018 to June 2024 were selected as the study objects.The microsatellite stable state of the patients was analyzed by immunohistochemistry and divided into microsatellite stable(MSS)group,low microsatellite unstable(MSI-L)group and high microsatellite unstable(MSI-H)group.The age,pathology and prognosis of the three groups were compared.Results:Among the 300 EC patients,218 patients(72.67%)were MSS patients,23 patients(7.67%)were MSI-L patients,and 59 patients(19.67%)were MSI-H patients.There were statistically significant differences in FIGO staging,carbohydrate antigen 125 level,lymphatic vessel invasion,lymphatic metastasis and P53 mutation among the three groups(P<0.05);there was no significant difference in age and differentiation degree among the three groups(P>0.05).The rate of poor prognosis in MSI-H group was higher than that in MSS group,the difference was statistically significant(P<0.001).There was no significant difference in the rate of poor prognosis between MSI-H group and MSI-L group(P=0.303).Conclusion:In the course of EC development,MMR protein deletion may be related to disease FIGO staging,lymphatic vessel invasion,lymph node metastasis,and carbohydrate antigen 125 expression,and MSI-H may also be one of the important factors affecting the poor prognosis of EC.
