摘　　要：眼肌麻痹是以眼珠突然偏斜、转动受限、视一为二为临床特征的眼病,与多种因素密切相关,严重影响患者的生活质量。中医病名为“风牵偏视”,其发病病因多纠于风、痰、瘀、虚,与肝、脾、肾三脏密切相关。目前,西医尚无特效药物及方法,多以控制原发病为主,对症予营养神经药物支持治疗。中医药在治疗该病并改善症状方面具有较好的临床疗效。中医学认为该病与阴阳不足、气血失调、肝脾不和、肝肾阴虚、痰湿凝滞、瘀血阻络等多种因素有关,主张治病求本,辨病辨证,四诊合参,相同的症状通过患者不同的体质,疾病不同的性质分别选方用药。接传红临床上应用“同病异治”治疗眼肌麻痹,“微著并重、辨证论治”,审证求因,四诊合参,取得了较好的临床疗效。笔者通过总结接传红治疗的3个经典眼肌麻痹案例阐释同病异治的实质,体现中医辨证论治的个体化特色,为提高中医临床诊疗水平提供思路。Ophthalmoplegia,characterized clinically by sudden ocular deviation,limited ocular movement,and diplopia,is closely associated with multiple factors and significantly impacts patients'quality of life.In Traditional Chinese Medicine(TCM),it is named"windinduced strabismus".From the perspective of TCM,its etiology is mostly attributed to wind,phlegm,blood stasis,and deficiency,with a close correlation to liver,spleen,and kidney organs.Currently,Western Medicine lacks specific drugs and treatments,mostly focusing on controlling the primary disease and providing symptomatic support with neurotrophic medications.TCM exhibits good clinical efficacy in treating this disease and improving symptoms.The disease is related to various factors such as Yin-Yang deficiency,Qi-blood imbalance,disharmony between liver and spleen,Yin deficiency of liver and kidney,phlegm-damp stagnation,and blood stasis obstructing the meridians.It advocates treating the root cause of the disease,differentiating diseases and syndromes,integrating the four diagnostic methods,and selecting prescriptions and medications based on patients'different constitutions and the nature of the disease despite similar symptoms.JIE Chuanhong clinically applies the principle of"treating the same disease with different methods"to the treatment of ophthalmoplegia,emphasizing both minor and significant symptoms,syndrome differentiation and treatment,examining the syndrome to determine the cause,and integrating the four diagnostic methods,achieving good clinical efficacy.The author summarizes three classic cases of ophthalmoplegia treated by JIE Chuanhong to illustrate the essence of treating the same disease with different methods,highlighting the individualized characteristics of syndrome differentiation and treatment in TCM,and providing ideas for improving the level of clinical diagnosis and treatment in TCM.
