摘 要:目的 探讨窄带成像联合放大内镜(NBI-ME)靶向活检在早期胃癌及癌前病变中的诊断价值。方法 选取2020年5月~2023年9月在马鞍山市人民医院消化内科就诊并行电子胃镜检查的患者360例,根据检查方式不同,将其分为窄带成像联合放大内镜(NBI-ME)组(n=120)和普通白光内镜(C-WLI)组(n=240)。C-WLI组使用C-WLI进行胃镜检查,对胃黏膜可疑病变部位进行活检;NBI-ME组先使用C-WLI进行胃镜检查,再行NBI-ME观察,对于可疑黏膜病变进行NBI-ME指导下靶向活检。记录两组患者病灶组织活检的病理结果。结果 NBI-ME组在判断病灶轮廓形态方面的作用显著优于C-WLI组,差异具有统计学意义(P <0.05);NBI-ME组早期胃癌和癌前病变的检出率分别为8.3%、33.3%,C-WLI组则分别为2.5%、17.9%,两组比较差异均有统计学意义(P <0.05)。结论 NBI-ME能够有效判断胃黏膜病变的边界,NBI-ME指导下靶向活检可以提高早期胃癌、癌前病变的检出率。Objective To investigate the diagnostic value of narrow band imaging combined with magnification endoscopy(NBI-ME)targeted biopsy in early gastric cancer and precancerous lesions.Methods A total of 360 patients who were admitted to the gastroenterology department of Maanshan People's Hospital from May 2020 to September 2023 and underwent electronic gastroscopy were selected,and divided into narrow band imaging combined with magnification endoscopy(NBI-ME)group(n=120)and common white light endoscopy(C-WLI)group(n=240)according to the different inspection method.In the C-WLI group,C-WLI was used for gastroscopy,and the suspected lesion of gastric mucosa was biopsied.In the NBI-ME group,C-WLI was first used for gastroscopy,followed by NBI-ME observation,and targeted biopsy under the guidance of NBI-ME was performed for suspected mucosal lesions.The pathological results of lesion biopsy were recorded in the two groups.Results The NBI-ME group was significantly better than that of C-WLI group in judging the contour shape of the lesion,and the difference was statistically significant(P<0.05).The detection rates of early gastric cancer and precancerous lesions were 8.3%and 33.3%in the NBI-ME group,respectively,while those in the C-WLI group were 2.5%and 17.9%,respectively,and the differences between the two groups were statistically significant(P<0.05).Conclusion NBI-ME can effectively determine the boundary of gastric mucosal lesions,and targeted biopsy under the guidance of NBI-ME can improve the detection rate of early gastric cancer and precancerous lesions.
