摘 要:目的研究联动成像内镜(linked color imaging,LCI)下大肠息肉内镜诊断分型与传统病理学分型的一致性并予以分析。方法选取江门市人民医院2021年3月至2022年12月收治的180例拟行结肠镜检查患者作为研究对象。将其以电脑编号奇偶数字法随机分作LCI检测组(n=90)和WLI检测组(n=90)。LCI检测组选用LCI进行检测,WLI检测组选用白光内镜(white light image,WLI)进行检测。对比两组大肠息肉检出率,分析不同直径、部位大肠息肉的检出率。此外,还进行LCI、WLI诊断分型与传统病理学分型一致性分析。结果LCI检测组大肠息肉总检出率高于WLI检测组(65.56%vs 34.44%,P<0.05),其中LCI检测组共检出大肠息肉161枚,WLI检测组共检出大肠息肉102枚。LCI检测组对直径1~3 mm大肠息肉的检出率高于WLI检测组(85.92%vs 68.75%,P<0.05)。LCI检测组对升结肠息肉的检出率高于WLI检测组(81.82%vs 53.33%),而降结肠息肉的检出率低于WLI检测组(50.00%vs 85.00%,均P<0.05)。LCI大肠息肉诊断分型与传统病理学分型的一致性极强(Kappa=0.954),WLI大肠息肉诊断分型与传统病理学分型的一致性较强(Kappa=0.771)。结论LCI对大肠息肉的检出率高于WLI,且诊断分型结果与传统病理学分型结果一致性极高。Objective To study the consistency of endoscopic diagnosis and traditional pathological classification of large intestine polyps under linked color imaging(LCI)and analyze it.Methods A total of 180 patients who were to undergo colonoscopy in Jiangmen People's Hospital from March 2021 to December 2022 were selected for the study.They were randomly divided into LCI detection group(n=90)and white light image(WLI)detection group(n=90)by computer numbered odd-even number method.LCI was used for detection in the LCI detection group,and WLI was used for detection in the WLI detection group.The detection rate of colorectal polyps in different diameters and positions was compared between the two groups.In addition,LCI and WLI diagnostic types were consistent with traditional pathologic types.Results The total detection rate of large intestine polyps in LCI detection group was higher than that in WLI detection group(65.56%vs 34.44%,P<0.05).161 polyps were detected in LCI detection group and 102 polyps in WLI detection group.The detection rate of 1-3 mm diameter polyps in LCI group was higher than that in WLI group(85.92%vs 68.75%,P<0.05).The detection rate of ascending colon polyps in LCI group was higher than that in WLI group(81.82%vs 53.33%),while the detection rate of descending colon polyps was lower than that in WLI group(50.00%vs 85.00%,all P<0.05).The LCI polyp diagnostic type was highly consistent with the traditional pathological type(Kappa=0.954),and the WLI polyp diagnostic type was strongly consistent with the traditional pathological type(Kappa=0.771).Conclusion The detection rate of large intestine polyps by LCI is higher than that by WLI,and the diagnostic classification results are very consistent with the traditional pathological classification results.
