终末期肾病血液透析患者焦虑、抑郁症状调查结果及其与睡眠质量、生命质量的相关性分析

摘　　要：目的:分析接受血液透析治疗的终末期肾病(ESRD)患者焦虑、抑郁情绪及与睡眠质量、生命质量之间的相关性。方法:选取2022年9月至2023年8月厦门市第五医院进行血液透析治疗的ESRD患者73例作为研究对象,以医院焦虑抑郁量表(HADS)评估患者情绪状态,以匹兹堡睡眠质量指数量表(PSQI)评估患者睡眠质量,以简易健康状况调查问卷(Mos SF-36)评估患者生命质量调查,分析焦虑、抑郁患者检出情况、得分情况以及患者睡眠质量、生命质量得分情况,分析焦虑、抑郁情绪与睡眠质量、生命质量相关性。结果:HADS-A≥8~≤10分者占49.32%,HADS-A>10分者占23.29%,HADS-D≥8~≤10分者占45.21%,HADS-D>10分者占21.92%。PSQI评分≥8分者占36.99%,SF-36<50分者占28.77%。HADS-A评分、HADS-D评分与PSQI评分正相关,与SF-36评分负相关(P<0.05)。结论:接受血液透析治疗的ESRD患者焦虑、抑郁情绪及与睡眠质量、生命质量之间存在相关性,其中,焦虑抑郁情绪与睡眠障碍正相关,与生命质量负相关。为了改善患者睡眠质量与生命质量有必要对其加强心理护理,改善其负性情绪,同时也有助于提高其治疗依从性。Objective:To investigate the correlations between anxiety,depression,and sleep quality with the quality of life in patients with end-stage renal disease(ESRD)undergoing hemodialysis.Methods:A total of 73 ESRD patients receiving hemodialysis at Xiamen Fifth Hospital from September 2022 to August 2023 were selected for this study.The Hospital Anxiety and Depression Scale(HADS)was used to assess the emotional status of patients,the Pittsburgh Sleep Quality Index(PSQI)was used to evaluate sleep quality,and the Medical Outcome Study 36-item Short Form(MOS SF-36)was used to assess quality of life.This study analyzed the prevalence and severity of anxiety and depression and evaluated their impacts on sleep quality and life quality,examining the correlations among these variables.Results:Among the patients,49.32%scored 8 to 10 on the HADS-A,23.29%scored above 10;similarly,45.21%scored 8 to 10 on the HADS-D,and 21.92%scored above 10.Additionally,36.99%had PSQI scores≥8,and 28.77%had SF-36 scores below 50.HADS-A and HADS-D scores were positively correlated with PSQI scores and negatively correlated with SF-36 scores(P<0.05).Conclusion:There is a correlation between anxiety,depression,sleep quality,and quality of life in ESRD patients undergoing hemodialysis.Anxiety and depression are positively correlated with sleep disturbances and negatively correlated with quality of life.Improving psychological care and effectively managing negative emotions may enhance sleep quality,overall life quality,and treatment adherence among these patients.
