天丹通络片联合降纤酶注射液治疗急性脑梗死的效果分析

摘　　要：目的:探究天丹通络片联合降纤酶(Defibrase,Df)注射液治疗急性脑梗死(Acute Cerebral Infarction,ACI)的效果。方法:回顾性收集我院2021年1月至2024年4月64例ACI患者病例资料,按用药方案不同分两组。以接受Df注射液治疗的32例患者列为单药组,以接受天丹通络片联合Df注射液治疗的32例患者列为联合组。对比两组疗效、美国国立卫生研究院卒中量表(National institute of health stroke scale,NIHSS)、蒙特利尔认知评估量表(Montreal cognitive assessment,MoCA)评分、脑血流动力学[特性阻抗(Characteristic impedance,ZCV)、平均血流量(Mean cerebrovascular blood flow,Qmean)、动态阻力(Dynamic resistance,DR)、平均血流速度(Mean flow velocity,Vmean)]、血清学指标水平、不良反应发生率。结果:联合组的总有效率明显高于单药组(P<0.05);治疗后,较之单药组,联合组的NIHSS评分、ZCV、DR水平及血清指标水平显著降低,MoCA评分、Qmean、Vmean水平显著升高(P<0.05);两组不良反应发生率无明显差异。结论:天丹通络片联合Df注射液治疗ACI患者不仅用药安全性高,且能进一步提升疗效,提高神经及认知功能,调节脑血流动力学水平,改善机体细胞因子水平。Objective:To explore the effect of Tiandan Tongluo Tablet combined with Defibrase(Df)injection in the treatment of acute cerebral infarction(ACI).Methods:The case data of 64 patients with ACI in our hospital from January 2021 to April 2024 were retrospectively collected and divided into two groups according to different medication regimens.The 32 patients received Df injection were listed as the single treatment group.The 32 patients received the treatment of Tian-Dan-Tong-Luo Tablet combined with Df injection were listed as the combined treatment group,and the efficacy,the National Institute of Health Stroke Scale(NIHSS),Montreal Cognitive Assessment(MoCA)scores,cerebral hemodynamic parameters[characteristic impedance(ZCV),mean cerebrovascular blood flow(Qmean),dynamic resistance(DR),mean flow velocity(Vmean)],serum indexes and adverse reaction rates of the two groups were compared.Results:The total effective rate of the combined treatment group was much higher than that of single treatment group(P<0.05).Compared with the single treatment group,after treatment,the combined treatment group had lower NIHSS score,levels of ZCV,DR,and serum indexes levels,higher MoCA score,levels of Qmean and Vmean(P<0.05);There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion:Tiandan Tongluo tablets combined with Df injection in the treatment of ACI patients is not only safe,but also can further improve the curative effect,neurological and cognitive function,regulate the level of cerebral hemodynamics,and improve the level of cytokines.
