上消化道溃疡出血患者外周血NLR、PLR、D-二聚体水平及鉴别诊断价值分析

摘　　要：目的:探讨上消化道溃疡出血患者外周血中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)、D-二聚体(D-D)水平及鉴别诊断价值。方法:选取2021年1月-2024年1月本院收治26例上消化道溃疡出血患者为出血组,以同期本院收治的34例非出血性急腹症患者为对照组,比较出血组和对照组患者NLR、PLR、D-D水平,采用受试者工作特征(ROC)曲线评价NLR、PLR、D-D和联合检测在上消化道溃疡出血中的价值。结果:出血组患者NLR、PLR高于对照组,D-D水平低于对照组(P<0.05)。NLR、PLR、D-D和联合检测在上消化道溃疡出血及非出血性急腹症鉴别诊断中的AUC分别为0.746、0.718、0.782、0.856(P<0.05),其中联合诊断的AUC最高。结论:上消化道溃疡出血患者外周血NLR、PLR较高,D-D水平较低,NLR、PLR、D-D联合检测能够提高上消化道溃疡出血及非出血性急腹症鉴别诊断价值。Objective:To explore the value of neutrophil/lymphocyte ratio(NLR)and platelet/lymphocyte ratio(PLR)combined with D-dimer(D-D)in the differential diagnosis of upper gastrointestinal ulcer bleeding and non-bleeding acute abdomen.Methods:A total of 26 patients with upper gastrointestinal ulcer bleeding and 34 patients with non-bleeding acute abdomen in the hospital were enrolled as bleeding group and non-bleeding group between January 2021 and January 2024.The levels of NLR,PLR and D-D in the two groups were detected.The value of NLR,PLR,D-D and combined detection in the differential diagnosis of upper gastrointestinal ulcer bleeding and non-bleeding acute abdomen was evaluated by receiver operating characteristic(ROC)curves.Results:NLR and PLR in bleeding group were higher than those in non-bleeding group,while D-D was lower than that in non-bleeding group(P<0.05).AUC values of NLR,PLR,D-D and combined detection in the differential diagnosis of upper gastrointestinal ulcer bleeding and non-bleeding acute abdomen were 0.746,0.718,0.782 and 0.856,respectively(P<0.05).AUC of combined detection was the greatest.Conclusion:The combined detection of NLR,PLR and D-D can improve the value in the differential diagnosis of upper gastrointestinal ulcer bleeding and non-bleeding acute abdomen.
