翼状胬肉切除术联合自体角膜缘干细胞移植术治疗翼状胬肉的有效性

摘　　要：目的观察翼状胬肉切除术联合自体角膜缘干细胞移植术治疗翼状胬肉的有效性。方法选取2020年1月-2023年1月湖口县人民医院诊治的60例翼状胬肉患者为研究对象,采用随机数字表法分为对照组(n=30)和观察组(n=30),对照组采用翼状胬肉切除术,观察组采用翼状胬肉切除术联合自体角膜缘干细胞移植术,比较两组视力、角膜散光度、主观症状评分、泪膜功能指标[泪膜破裂时间(BUT)和角膜荧光素染色(CFS)评分]、并发症发生率、复发率。结果两组术后视力均大于术前,角膜散光度均小于术前,且观察组视力大于对照组,角膜散光度小于对照组(P<0.05);观察组术后3、6 d主观症状评分均低于对照组(P<0.05);两组术后BUT均大于术前,CFS均小于术前,且观察组BUT大于对照组,CFS小于对照组(P<0.05);观察组并发症发生率为6.67%,低于对照组的20.00%(P<0.05);观察组复发率为3.33%,低于对照组的13.33%(P<0.05)。结论翼状胬肉切除术联合自体角膜缘干细胞移植术治疗翼状胬肉具有确切的效果,可有效改善泪膜稳定性及泪膜功能,促进视力改善,减小角膜散光度,且可有效降低并发症发生率和复发率。Objective To observe the efficacy of pterygium excision combined with autologous limbal stem cell transplantation in the treatment of pterygium.Methods From January 2020 to January 2023,60 patients with pterygium diagnosed and treated in Hukou County People's Hospital were selected as the research objects.They were divided into control group(n=30)and observation group(n=30)by random number table method.The control group was treated with pterygium excision,and the observation group was treated with pterygium excision combined with autologous limbal stem cell transplantation.The visual acuity,corneal astigmatism,subjective symptom score,tear film function index[tear film rupture time(BUT)and corneal fluorescein staining(CFS)score],complication rate and recurrence rate were compared between the two groups.Results The postoperative visual acuity of the two groups was greater than that before operation,the corneal astigmatism was smaller than that before operation,and the visual acuity of the observation group was greater than that of the control group,the corneal astigmatism was smaller than that of the control group(P<0.05).The score of subjective symptom in the observation group was lower than that in the control group at 3 and 6 days after operation(P<0.05).The BUT of the two groups after operation was larger than that before operation,the CFS was smaller than that before operation,and the BUT of the observation group was larger than that of the control group,the CFS was smaller than that of the control group(P<0.05).The incidence of complications in the observation group was 6.67%,which was lower than 20.00%in the control group(P<0.05).The recurrence rate of the observation group was 3.33%,which was lower than 13.33%of the control group(P<0.05).Conclusion Pterygium excision combined with autologous limbal stem cell transplantation has a definite effect in the treatment of pterygium,which can effectively improve tear film stability and tear film function,promote visual acuity improvement,reduce corneal astigmatism
