不同剂量奥美拉唑治疗十二指肠球部溃疡的临床疗效及对其胃内pH值的影响

摘　　要：目的探讨不同剂量奥美拉唑治疗十二指肠球部溃疡的临床疗效及对其胃内pH值的影响。方法选取乐安县人民医院2020年7月-2023年6月收治的72例十二指肠球部溃疡患者作为研究对象,按随机数字表法分为对照组和研究组,每组36例。对照组给予常规剂量奥美拉唑治疗,研究组给予大剂量奥美拉唑治疗,比较两组患者临床疗效、免疫功能[免疫球蛋白M(IgM)、免疫球蛋白A(IgA)、免疫球蛋白G(IgG)]、胃内pH变化情况(24 h内胃内平均pH、pH值到达4.0和6.0所需时间及达到后的持续时间)、炎性因子水平[肿瘤坏死因子-α(TNF-α)、白介素-8(IL-8)、白介素-17(IL-17)]。结果研究组治疗总有效率高于对照组(P<0.05)。两组治疗后患者IgM、IgA、IgG水平均升高,且研究组IgM、IgA、IgG水平高于对照组(P<0.05)。两组患者的24 h内胃内平均pH值、pH值到达4.0及6.0持续时间比较,差异无统计学意义(P>0.05);而研究组患者pH值到达4.0、pH值到达6.0所需时间高于对照组(P<0.05)。两组治疗后TNF-α、IL-8、IL-17水平均较治疗前降低,且研究组低于对照组(P<0.05)。结论大剂量治疗十二指肠球部溃疡患者的效果确切,能够更快改善患者胃内pH值,提高患者机体免疫功能,降低炎性因子水平,值得临床应用。Objective To investigate the clinical efficacy of different doses of omeprazole in the treatment of duodenal ulcer and its effect on gastric pH.Methods A total of 72 patients with duodenal ulcer admitted to Le′an County People′s Hospital from July 2020 to June 2023 were selected as the research objects.According to the random number table method,they were divided into control group and study group,with 36 patients in each group.The control group was treated with conventional dose of omeprazole,and the study group was treated with high-dose omeprazole.The clinical efficacy,immune function[immunoglobulin M(IgM),immunoglobulin A(IgA),immunoglobulin G(IgG)],gastric pH changes(average gastric pH within 24 hours,time required for pH to reach 4.0 and 6.0,and duration after reaching),inflammatory factor levels[tumor necrosis factor-α(TNF-α),interleukin-8(IL-8),interleukin-17(IL-17)]were compared between the two groups.Results The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).After treatment,the levels of IgM,IgA and IgG in the two groups increased,and the levels of IgM,IgA and IgG in the study group were higher than those in the control group(P<0.05).There was no significant difference in the average pH value in the stomach within 24 hours and the duration of pH value reaching 4.0 and 6.0 between the two groups(P>0.05).The time required for the pH value to reach 4.0 and the pH value to reach 6.0 in the study group was higher than that in the control group(P<0.05).After treatment,the levels of TNF-α,IL-8 and IL-17 in the two groups were lower than those before treatment,and those in the study group were lower than those in the control group(P<0.05).Conclusion High-dose treatment of patients with duodenal bulb ulcer has a definite effect,which can improve the gastric pH value and immune function,reduce the level of inflammatory factors.It is worthy of clinical application.
