摘  要:目的探讨分期取穴针刺法结合表面肌电生物反馈(sEMGBF)对脑卒中后吞咽障碍患者吞咽功能的影响。方法选择郑州市第十五人民医院在2021年1月至2023年1月收治的80例脑卒中后吞咽障碍患者作为研究对象,将其随机划分为观察组和对照组,各40名。对照组给予sEMGBF法治疗,观察组在对照组基础上给予分期取穴针刺法治疗。比较两组洼田饮水试验(WST)、表面肌电图(sEMG)检测结果、吞咽疗效、吞咽功能、生活质量。结果治疗后,两组患者WST分级均明显改善,且观察组优于对照组,差异有统计学意义(P<0.05);与治疗前比较,两组患者sEMG平均波幅升高,且观察组高于对照组,差异有统计学意义;两组患者吞咽时限降低,且观察组低于对照组,差异有统计学意义(P<0.05);观察组疗后有效率显著高于对照组,差异有统计学意义(P<0.05);与治疗前比较,两组患者SSA评分均降低,且观察组降低幅度更大,差异有统计学意义(P<0.05);两组患者较治疗前SWAL-QOL评分均升高,且观察组上升幅度更大,差异有统计学意义(P<0.05)。结论对脑卒中后吞咽障碍患者给予分期取穴针刺法结合表面肌电生物反馈治疗,可明显改善患者吞咽功能,提高生活质量,获得良好恢复效果,值得应用。Objective To investigate the effects of staged acupoint needle insertion combined with surface electromyography biofeedback(sEMGBF)on swallowing function in patients with dysphagia after stroke.Methods This study selected 80 patients with post-stroke dysphagia treated at Zhengzhou Fifteenth People's Hospital from January 2021 to January 2023.Patients were randomly divided into an observation group and a control group,with 40 patients in each group.The control group received sEMGBF treatment,while the observation group received staged acupoint needle insertion in addition to sEMGBF.Swallowing function was evaluated using the Water Swallowing Test(WST),sEMG measurements,swallowing efficacy,and quality of life.Results After treatment,WST scores improved significantly in both groups,with the observation group showing better outcomes than the control group(P<0.05).The average amplitude of sEMG increased in both groups compared to before treatment,with the observation group demonstrating a significantly higher amplitude than the control group(P<0.05).The duration of swallowing was reduced in both groups,with the observation group showing a shorter duration than the control group(P<0.05).The effective treatment rate in the observation group was significantly higher than in the control group(P<0.05).SSA(Swallowing Severity Assessment)scores decreased in both groups compared to baseline,with a more significant reduction in the observation group(P<0.05).Additionally,SWAL-QOL(Quality of Life in Swallowing Disorders)scores increased in both groups,with the observation group showing a greater improvement(P<0.05).Conclusion The combination of staged acupoint needle insertion and surface electromyography biofeedback significantly improves swallowing function and enhances quality of life in patients with post-stroke dysphagia,leading to positive recovery outcomes,and is worthy of clinical application.
