自发性脑出血病因学诊断中应用头颈部CT血管造影成像的临床价值分析

摘　　要：目的探究病因学诊断中,对于自发性脑出血病例应用头颈部计算机断层成像血管造影(CTA)的价值。方法选取南阳市中心医院2022年1月至2024年1月期间收治的92例自发性脑出血患者,研究样本均进行CTA和数字减影血管造影(DSA)检查,同时以DSA检查结果为金标准,分析CTA的诊断情况,同时评估其在预测自发性脑出血的价值。结果两种方式诊断自发性脑出血疾病阳性检出率比较,差异无统计学意义(P>0.05)。其中CTA敏感度为96.25%(77/80),特异度为75.00%(9/12)。CTA预测血肿扩大方面,斑点征阳性的准确率为84.62%;在CTA预测血肿未扩大方面斑点征阴性的准确率为97.50%。结论CTA在诊断自发性脑出血中具有较高的诊断率,能明确脑出血的病因,且对血肿扩大具有预测价值,具有临床应用意义。Objective To explore the value of head and neck computed tomography angiography(CTA)in the etiological diagnosis of spontaneous intracerebral hemorrhage cases.Methods A total of 92 patients with spontaneous intracerebral hemorrhage admitted to Nanyang Central Hospital from January 2022 to January 2024 were selected for the study.All samples underwent both CTA and digital subtraction angiography(DSA),with DSA results serving as the gold standard for analysis of CTA diagnostic performance,as well as its predictive value for spontaneous intracerebral hemorrhage.Results There was no statistically significant difference in the positive detection rates for spontaneous intracerebral hemorrhage between the two methods(P>0.05).The sensitivity of CTA was 96.25%(77/80),and the specificity was 75.00%(9/12).Regarding the prediction of hematoma expansion,the accuracy for positive dot sign(indicating expansion)was 84.62%,while the accuracy for negative dot sign(indicating no expansion)was 97.50%in predicting non-expansion of the hematoma.Conclusion CTA demonstrates a high diagnostic rate for spontaneous intracerebral hemorrhage and effectively clarifies the underlying causes of the hemorrhage.It also holds predictive value for hematoma expansion,indicating its clinical significance.
