摘　　要：目的:分析同型半胱氨酸(Hcy)、脂蛋白a[LP(a)]、脑钠肽(BNP)、超敏C反应蛋白(hs-CRP)在进展性缺血性脑卒中(PIS)诊断中的应用价值。方法:将2023年1—12月徐州矿务集团第二医院收治的82例PIS患者纳入试验组,根据颈动脉硬化厚度将其分为正常组与增厚组,根据臂踝脉搏波传导速度(baPWV)将其分为轻度硬化组、中度硬化组、重度硬化组;选取同期健康体检者82例作为对照组。检测、比较对照组和试验组、试验组各个亚组血清Hcy、LP(a)、BNP、hs-CRP水平,分析PIS患者baPWV与各项血清指标的相关性。结果:试验组血清Hcy、LP(a)、BNP、hs-CRP水平高于对照组(P<0.05)。增厚组血清Hcy、LP(a)、BNP、hs-CRP水平高于正常组(P<0.05)。不同baPWV PIS患者血清Hcy、LP(a)、BNP、hs-CRP水平比较,轻度组<中度组<重度组(P<0.05)。PIS患者baPWV与Hcy、LP(a)、BNP、hs-CRP水平呈正相关(P<0.05)。结论:PIS患者血清Hcy、LP(a)、BNP、hs-CRP表达水平较高,且随着颈动脉内膜增厚及硬化程度加重不断升高,上述指标水平可有效反映PIS患者病情及动脉粥样硬化严重程度,为临床诊治提供可靠依据。Objective:To analyze the application value of homocysteine(Hcy),lipoprotein a[LP(a)],brain natriuretic peptide(BNP),hypersensitive C-reactive protein(hs-CRP)in the diagnosis of progressive ischemic stroke(PIS).Methods:A total of 82 pa⁃tients with PIS admitted to the Second Hospital of Xuzhou Coal Mining Group from January 2023 to December 2023 were included into the experimental group.According to the thickness of carotid atherosclerosis,they were divided into a normal group and a thickened group.According to the brachial ankle pulse wave velocity(baPWV),they were divided into a mild sclerosis group,a moderate sclerosis group and a severe sclerosis group.A total of 82 healthy people who underwent health checkups during the same period were selected as the control group.The levels of serum Hcy,LP(a),BNP,and hs-CRP between the control group and the experimental group and among different subgroups of the experimental group were detected and compared.The correlation be⁃tween baPWV and various serum indexes in PIS patients was analyzed.Results:The levels of serum Hcy,LP(a),BNP and hs-CRP in the experimental group were higher than those in the control group(P<0.05).The levels of serum Hcy,LP(a),BNP and hs-CRP in the thickened group were higher than those in the normal group(P<0.05).The levels of serum Hcy,LP(a),BNP and hs-CRP in PIS patients with different baPWVs were compared and ranked as follows:mild group<moderate group<severe group(P<0.05).The baPWV was positively correlated with the levels of Hcy,LP(a),BNP and hs-CRP in PIS patients(P<0.05).Conclusion:The expression levels of serum Hcy,LP(a),BNP and hs-CRP in PIS patients are relatively high,and the levels continuously increase with the thickening of carotid intima and the severity of sclerosis.The above indexes can effectively reflect the disease condition and the severity of atherosclerosis in PIS patients,and provide a reliable basis for clinical diagnosis and treatment.
