阿立哌唑联合艾司西酞普兰治疗重度抑郁症的效果及对血清皮质醇水平的影响

摘　　要：目的分析阿立哌唑联合艾司西酞普兰治疗重度抑郁症的效果及对血清皮质醇水平的影响。方法回顾我院2020年1月-2023年12月内收治的54例重度抑郁症患者为本次研究对象,所有患者按治疗方案分组,分为单药组(n=27,艾司西酞普兰治疗)和两联组(n=27,阿立哌唑+艾司西酞普兰治疗)。统计对比两组患者临床疗效及治疗前后的血生化指标、心理健康状态评分、睡眠质量评分、社会功能缺陷评分,并统计对比两组患者治疗期间药物不良反应的发生率。结果两组治疗期间药物不良反应发生率组间对比,无统计学差异(P<0.05),但两联组患者治疗总有效率显著高于单药组(P<0.05),且治疗后两联组血清Cor及SCL-90、PSQI、SDSS评分均显著低于单药组(P<0.05),血清BDNF、NT-3、5-HT、DA均显著高于单药组(P<0.05)。结论重度抑郁症患者以阿立哌唑+艾司西酞普兰联合治疗,可有效调节神经递质,进而改善患者的心理健康状态、睡眠质量与社会功能缺陷,提高生活质量,疗效显著,且不会产生严重不良反应,倡导临床应用。Objective To analyze the efficacy of aripiprazole combined with escitalopram in the treatment of major depressive disorder and the effects on serum cortisol levels.Methods 54 patients with major depressive disorder admitted to our hospital from January 2020 to December 2023 were reviewed as the subjects of this study.All patients were divided into single drug group(n=27,escitalopram treatment)and dual group(n=27,aripiprazole+Escitalopram treatment)according to the treatment regimen.The clinical efficacy,blood biochemical indexes,mental health status scores,sleep quality scores and social dysfunction scores before and after treatment were statistically compared between the two groups,and the incidence of side effects of drugs during treatment was statistically compared between the two groups.Results There was no statistically significant difference in the incidence of side effects during treatment between the two groups(P<0.05),but the treatment effective rate in the dual groups was significantly higher than that in the single drug group(P<0.05),and after treatment,serum Cor,SCL-90,PSQI and SDSS scores in the dual group were significantly lower than those in the single drug group(P<0.05),serum BDNF,NT-3,5-HT and DA were significantly higher than those in single drug group(P<0.05).Conclusion The combined treatment of aripiprazole and escitalopram in patients with major depression can effectively regulate neurotransmitters,so as to improve mental health,sleep quality and social dysfunction,and improve quality of life,so as to achieve significant curative effect without causing serious adverse reactions,so clinical application is advocated.
