摘　　要：目的对比容积旋转调强放疗(VMAT)、螺旋断层放疗(HT)治疗局部晚期喉癌的效果。方法82例局部晚期喉癌患者随机分为参照组(VMAT治疗)和研究组(HT治疗),比较两组的剂量学参数和肿瘤标志物水平。结果研究组GTV与PTV的D2、D50、HI低于参照组,CI高于参照组,治疗后的血清CA125、CA19-9、CEA水平低于参照组(P<0.05)。结论与VMAT相比,HT治疗局部晚期喉癌患者GTV与PTV放疗药物剂量的适形度、均匀性更好,使用剂量更少,肿瘤标志物水平更低。Objective To compare the effects of volumetric modulated arc therapy(VMAT)and helical tomotherapy(HT)in the treatment of locally advanced laryngeal cancer.Methods 82 patients with locally advanced laryngeal cancer were randomly divided into the reference group(VMAT treatment)and the study group(HT treatment).The dosimetric parameters and tumor marker levels of the two groups were compared.Results The D_2,D_(50)and HI of GTV and PTV in the study group were lower than those in the reference group,the CI were higher than those in the reference group,and the serum CA125,CA19-9 and CEA levels after treatment were lower than those in the reference group(P<0.05).Conclusions Compared with VMAT,HT in the treatment of patients with locally advanced laryngeal cancer has better conformal and uniformity of GTV and PTV radiotherapy drug dose,lower use of dose,and lower levels of tumor markers.
