外周血T淋巴细胞亚群、IL-37联合PNI对多发性骨髓瘤患者化疗效果及预后的预测价值

摘　　要：目的探讨外周血T淋巴细胞亚群、白细胞介素-37(IL-37)联合预后营养指数(PNI)对多发性骨髓瘤(MM)患者化疗效果及预后的预测价值。方法选取2020年01月~2023年01月入院治疗的103例MM患者作为研究对象,采集治疗前患者外周血T淋巴细胞亚群、IL-37、PNI,探讨接受化疗的MM患者外周血T淋巴细胞亚群、IL-37、PNI对短期疗效、预后的评估价值。结果CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、IL-37、PNI均为MM患者化疗疗效的独立影响因素(P<0.05)。CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、IL-37、PNI曲线下面积(AUC)分别为0.719(95%CI:0.619~0.820)、0.764(95%CI:0.670~0.858)、0.732(95%CI:0.625~0.840)、0.700(95%CI:0.598~0.802)、0.779(95%CI:0.689~0.869)、0.750(95%CI:0.654~0.845),联合预测AUC为0.852(95%CI:0.777~0.926)。结论外周血T淋巴细胞亚群、IL-37、PNI水平为MM患者化疗疗效的独立影响因素,并且对MM患者预后具有较好的诊断效能。Objective Objective:To explore the predictive value of peripheral blood T lymphocyte subsets,interleukin-37(IL-37)combined with prognostic nutritional index(PNI)for chemotherapy efficacy and prognosis in patients with multiple myeloma(MM).Methods 103 MM patients admitted for treatment from January 2020 to January 2023 were selected as the research subjects.Peripheral blood T lymphocyte subsets,IL-37,and PNI were collected before treatment to explore the evaluation value of peripheral blood T lymphocyte subsets,IL-37,and PNI for short-term efficacy and prognosis in MM patients receiving chemotherapy.Results CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),IL-37,and PNI were all independent influencing factors of chemotherapy efficacy in MM patients(P<0.05).CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),IL-37,and area under the PNI curve(AUC)were 0.719(95%CI:0.619~0.820),0.764(95%CI:0.670~0.858),0.732(95%CI:0.625~0.840),0.700(95%CI:0.598~0.802),0.779(95%CI:0.689~0.869),and 0.750(95%CI:0.654~0.845),respectively.The combined predicted AUC was 0.852(95%CI:0.777~0.926).Conclusions Peripheral blood T lymphocyte subsets,IL-37,and PNI levels are independent influencing factors of chemotherapy efficacy in MM patients,and have good diagnostic efficacy for the prognosis of MM patients.
