半夏泻心汤加减联合四联疗法对幽门螺杆菌相关性胃炎氧化应激及血清miR-155、sIL-2R的影响

摘　　要：目的:观察半夏泻心汤加减联合四联疗法对幽门螺杆菌(Hp)相关性胃炎患者氧化应激及血清微小核糖核酸-155(miR-155)、可溶性白细胞介素-2受体(sIL-2R)的影响。方法:选取94例Hp相关性胃炎患者,按随机数字表法分为对照组及研究组各47例。对照组给予四联疗法治疗,研究组给予四联疗法联合半夏泻心汤加减治疗。比较2组临床疗效及Hp转阴率、不良反应发生率,比较2组治疗前后中医证候积分、氧化应激指标值[一氧化氮(NO)、丙二醛(MDA)、超氧化物歧化酶(SOD)]、血清miR-155、sIL-2R水平的变化。结果:研究组临床疗效总有效率为91.49%,对照组为74.47%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组中医证候积分均较治疗前下降(P<0.05),研究组中医证候积分低于对照组(P<0.05)。治疗后,2组NO、MDA指标值均较治疗前下降(P<0.05),SOD指标值均较治疗前上升(P<0.05);研究组NO、MDA指标值均低于对照组(P<0.05),SOD指标值高于对照组(P<0.05)。治疗后,2组血清miR-155相对表达量、sIL-2R水平均较治疗前下降(P<0.05),研究组血清miR-155相对表达量、sIL-2R水平均小于对照组(P<0.05)。研究组Hp转阴率为87.23%,对照组Hp转阴率为68.09%,2组Hp转阴率比较,差异有统计学意义(P<0.05)。研究组不良反应发生率为6.38%,对照组不良反应发生率为8.51%,2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:半夏泻心汤加减联合四联疗法治疗Hp相关性胃炎可有效缓解症状,减轻应激反应,提高Hp转阴率及临床疗效,安全性高。Objective:To observe the effects of modified Banxia Xiexin Decoction combined with quadruple therapy on oxidative stress,serum microRNA-155(miR-155)and soluble interleukin-2 receptor(sIL-2R)in patients with Helicobacter pylori(Hp)-associated gastritis.Methods:A total of 94 cases of patients with Hp-associated gastritis were selected and divided into the control group and the study group according to the random number table method,with 47 cases in each group.The control group was treated with quadruple therapy,and the study group was treated with quadruple therapy combined with modified Banxia Xiexin Decoction.The clinical effects,Hp negative rates and incidence of adverse reactions were compared between the two groups;traditional Chinese medicine syndrome scores,oxidative stress index values[nitric oxide(NO),malondialdehyde(MDA),superoxide dismutase(SOD)],serum miR-155 and sIL-2R levels were compared between the two groups before and after treatment.Results:The total effective rate was 91.49%in the study group and 74.47%in the control group,the difference being significant(P<0.05).After treatment,TCM syndrome scores in the two groups were decreased when compared with those before treatment,and the TCM syndrome score in the study group was lower than that in the control group(P<0.05).After treatment,the values of NO and MDA in the two groups were decreased when compared with those before treatment(P<0.05),and the values of SOD were increased when compared with those before treatment(P<0.05);the indexes of NO and MDA in the study group were lower than those in the control group(P<0.05),and the SOD index in the study group was higher than that in the control group(P<0.05).After treatment,the relative expressions of serum miR-155 and the levels of sIL-2R in the two groups were reduced when compared with those before treatment(P<0.05),and the relative expression of serum miR-155 and the level of sIL-2R in the study group were lower than those in the control group(P<0.05).The Hp negative rate was 87.23%in the study gro
