清热泻火解毒汤联合常规西药治疗慢性牙周炎胃火炽盛证临床研究

摘　　要：目的:观察清热泻火解毒汤联合常规西药治疗慢性牙周炎胃火炽盛证的临床疗效。方法:选取84例慢性牙周炎患者为研究对象,按随机数字表法分为联合组与参照组各42例。参照组采用常规西药治疗,联合组在参照组基础上加用清热泻火解毒汤治疗。治疗4个疗程后比较2组临床疗效,并比较2组治疗前后中医证候积分、牙周组织损伤、龈沟液炎症反应。结果:联合组临床总有效率92.86%,高于参照组71.43%(P<0.05)。治疗后,2组中医证候积分均降低(P<0.05),且联合组低于参照组(P<0.05)。治疗后,2组牙菌斑指数、牙周袋深度、牙龈指数均降低(P<0.05),且联合组低于参照组(P<0.05)。治疗后,2组龈沟液白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)水平均降低(P<0.05),且联合组低于参照组(P<0.05)。结论:清热泻火解毒汤联合常规西药治疗慢性牙周炎胃火炽盛证患者疗效确切,可有效缓解症状,修复患者牙周组织,减轻龈沟液炎症反应。Objectives:To observe the clinical effect of Qingre Xiehuo Jiedu Decoction combined with conventional western medicine on chronic periodontitis with intense stomach fire syndrome.Methods:A total of 84 cases of patients with chronic periodontitis were divided into the combination group and the reference group according to the random number table method,with 42 cases in each group.The reference group was treated with conventional western medicine,and the combination group was additionally treated with Qingre Xiehuo Jiedu Decoction based on the treatment of the reference group.After four treatment courses,clinical effects were compared between the two groups.Traditional Chinese medicine syndrome scores,periodontal tissue injury and gingival crevicular fluid inflammatory responses were compared between the two groups.Results:The total clinical effective rate was 92.86%in the combination group,higher than that of 71.43%in the reference group(P<0.05).After treatment,traditional Chinese medicine syndrome scores in the two groups were decreased(P<0.05),and the scores in the combination group was lower than that in the reference group(P<0.05).After treatment,plaque indexes,periodontal pocket depth,and gingival indexes in the two groups were decreased(P<0.05),and the above indexes in the combination group were lower than those in the reference group(P<0.05).After treatment,the levels of interleukin-6(IL-6)and interleukin-1β(IL-1β)in gingival crevicular fluid in the two groups were decreased(P<0.05),and the above levels in the combination group were lower than those in the reference group(P<0.05).Conclusion:Qingre Xiehuo Jiedu Decoction combined with conventional western medicine has adefinite curative effect in the treatment of chronic periodontitis with intense stomach fire syndrome and caneffectively relieve symptoms,repair the patient's periodontal tissue and reduce inflammation in gingivalcrevicular fluid.
