儿科住培生指南知晓依从情况及影响因素的调查研究

摘  要:目的探讨参加北京儿童医院住院医师规范化培训的医学生(住培生)对指南的知晓、依从情况及其影响因素,提出针对性教学管理建议。方法2022年10月通过问卷法调查2020至2022年三届北京儿童医院30个科室住培生共计264人,采用Likert 5级评分法衡量儿科住培生对指南的知晓及依从现状,并采用单因素分析及多元线性回归探究其影响因素。结果儿科住培生对指南的知晓情况为2.77±0.836分,处于中等偏差水平;对阅读指南的积极性为3.77±0.752分,处于中等偏上水平;依从情况较为乐观。进一步分析后表明:住培年限、临床工作经历、指南阅读经历和阅读积极性是影响知晓情况的主要因素;缺乏指导、获取困难和工作原因是儿科住培生在阅读指南过程中的主要困难;获取困难和实施受限是其依从的主要阻碍因素。结论儿科住培生对指南的知晓有待加强,可通过加强重视循证医学教育、提供专业的指导和获取途径、增加多角度接触指南的机会、提供自主学习的时间等方式增加儿科住培生对指南阅读的积极性和自觉性;对指南的依从性较为乐观,可通过遴选适用性较高的临床指南以增加儿科住培生对指南的依从性。Objective To investigate and analyze the level of understanding,compliance,and influencing factors among medical students(resident trainees)participating in standardized residency training at Beijing Children's Hospital and propose specific teaching and management recommendations.Methods In October 2022,a questionnaire survey was conducted among 264 pediatric residents from 30 departments at Beijing Children's Hospital,covering the 2020 to 2022 cohorts.The study utilized a Likert 5-point scale to assess the residents'awareness and compliance with the training guidelines.Single-factor analysis and multiple linear regression were employed to explore the influencing factors.Results Pediatric residents’understanding of the guidelines was 2.77±0.836 points,at the medium deviation level.Their enthusiasm to read guidelines was 3.77±0.752 points,above the medium level,and their compliance was relatively optimistic.Further analysis showed that years of residency training,clinical work experience,guideline reading experience,and enthusiasm to read were the main negative factors that affected their knowledge of guidelines.Lack of guidance and difficulties obtaining and working are the main obstacles for resident students in reading guidelines.Difficulties in access and limited implementation are the main obstacles to compliance.Conclusions The degree of understanding in pediatric students needs to be strengthened,and the initiative and consciousness of pediatric students in reading guidelines can be increased by emphasizing evidence-based medicine education,providing professional guidance and access,increasing the opportunities to contact the guidelines from multiple perspectives,and providing time for independent study.Adherence to guidelines is optimistic,and we recommended increasing adherence among pediatric residents by selecting clinical guidelines with high applicability and promoting their dissemination.
