艾地苯醌片联合经颅磁刺激治疗缺血性脑卒中后抑郁的临床效果

摘　　要：目的观察艾地苯醌片联合经颅磁刺激治疗缺血性脑卒中后抑郁的临床效果。方法选择2021年2月—2022年8月天水市中医医院收治的缺血性脑卒中后抑郁患者104例,根据随机数字表法分为观察组和对照组各52例。对照组给予常规盐酸氟西汀胶囊治疗,观察组在对照组基础上给予艾地苯醌片口服联合经颅磁刺激治疗。治疗2周后比较2组疗效,治疗前后汉密尔顿抑郁量表(HAMD)评分及5-羟色胺(5-HT)、去甲肾上腺素(NE)、多巴胺(DA)水平。结果观察组治疗总有效率为92.31%,高于对照组的76.92%(χ^(2)=4.727,P=0.030)。治疗2周后,2组HAMD评分均低于治疗前,且观察组低于对照组(P<0.01);2组5-HT、DA、NE水平均高于治疗前,且观察组高于对照组(P<0.01)。结论艾地苯醌片联合经颅磁刺激治疗缺血性脑卒中后抑郁效果较好,可有效减轻患者临床症状,改善神经递质水平。Objective To observe the clinical effect of idebenone tablets combined with transcranial magnetic stimulation in the treatment of depression after ischemic stroke.Methods A total of 104 patients with depression after ischemic stroke admitted to Tianshui Hospital of Traditional Chinese Medicine from February 2021 to August 2022 were selected and divided into observation group and control group with 52 patients in each group according to random number table method.The control group received routine treatment with fluoxetine hydrochloride capsules,and the observation group was treated with idebenone tablets combined with transcranial magnetic stimulation on the basis of the control group.After 2 weeks of treatment,the efficacy,the HAMD scores and the levels of 5-HT,NE and DA before and after treatment in both groups were compared.Results The total effective rate of the observation group was 92.31%,higher than 76.92%of the control group(χ^(2)=4.727,P=0.030).After 2 weeks of treatment,HAMD scores in two groups were lower than before treatment,and observation group was lower than control group(P<0.01).The levels of 5-HT,DA and NE in two groups were higher than before treatment,and the observation group was higher than control group(P<0.01).Conclusion Idebenone tablets combined with transcranial magnetic stimulation has a good effect in the treatment of depression after ischemic stroke,which can effectively reduce clinical symptoms and improve neurotransmitter levels.
