探讨冰冻对肺不典型腺瘤样增生病变的诊断价值

摘　　要：目的目的探讨术中冰冻对于肺不典型腺瘤样增生的诊断价值。方法方法回顾性分析2023年6月到2024年10月送至空军军医大学第二附属医院病理科做术中冰冻检查,经过术中冰冻制片阅片,冰冻回报结果为不典型腺瘤性增生,待术后石蜡进一步明确检查病例数202例,对比术后石蜡切片结果,其中原位腺癌136例,占比67.33%;微浸润癌18例,占比8.91%,腺鳞癌1例,占比0.50%;石蜡结果仍符合不典型腺瘤性增生的47例,占比23.27%。结果结果术中冰冻诊断为肺不典型腺瘤样增生,术后石蜡结果大多为原位腺癌,小部分微浸润癌,结果仍符合不典型腺瘤样增生的所占比例较低。结论结论术中冰冻切片对于肺不典型腺瘤样增生和原位腺癌较难鉴别诊断,传统石蜡诊断对于此类诊断更准确。Objective To explore the diagnostic value of intraoperative freezing for atypical adenomatous hyperplasia of the lung.Method A retrospective analysis was conducted on 202 cases of atypical adenomatous hyperplasia who were sent to the Pathology Department of the Second Affiliated Hospital of the Air Force Medical University for intraoperative frozen examination from June 2023 to October 2024.After intraoperative frozen section reading,the frozen report showed atypical adenomatous hyperplasia.After further examination with paraffin after surgery,the number of cases was 202.The results of postoperative paraffin section were compared,among which 136 cases were in situ adenocarcinoma,accounting for 67.33%;18 cases of microinvasive carcinoma,accounting for 8.91%,and 1 case of adenosquamous cell car-cinoma,accounting for 0.50%;The paraffin results still correspond to 47 cases of atypical adenomatous hyperplasia,accounting for 23.27%.Result The intraoperative frozen diagnosis was atypical adenomatous hyperplasia of the lung,and the postoperative paraffin results were mostly in situ adenocarcinoma,with a small amount of microinvasive carcinoma.The proportion of results consistent with atypical adenoma-tous hyperplasia was relatively low.Conclusion Intraoperative frozen sections are difficult to differentiate between atypical adenomatous hyperplasia and in situ adenocarcinoma of the lung,while traditional paraffin diagnosis is more accurate for such cases.
