摘 要:目的探究食管鳞状细胞癌(esophageal squamous cell carcinoma,ESCC)中壳多糖酶3样蛋白1(chitinase-3-like protein 1,CHI3L1)与Wnt/β-catenin信号通路相关分子的关系。方法回顾性选取119例ESCC石蜡组织标本及60例正常食管组织标本。采用免疫组化EliVision Plus法检测上述组织中CHI3L1及Wnt/β-catenin信号通路相关分子的蛋白表达水平,比较ESCC患者组织和正常食管组织中CHI3L1、TCF4、Cyclin D1、APC表达及β-catenin蛋白异常表达水平,并随访ESCC患者的生存情况。结果与正常食管组织比较,ESCC中CHI3L1、TCF4、Cyclin D1阳性率及β-catenin异常阳性率升高(分别为63.0%、69.7%、68.1%、67.2%),APC阳性率降低(41.2%)。CHI3L1、TCF4、Cyclin D1、β-catenin四种蛋白表达水平之间均呈显著正相关(P<0.05),而与APC表达呈负相关(P<0.05)。CHI3L1蛋白与ESCC肿瘤大小、浸润深度、淋巴结转移和pTNM分期相关(P<0.05)。CHI3L1、TCF4、Cyclin D1蛋白表达或β-catenin蛋白异常表达与ESCC患者预后不良相关,提示患者生存时间缩短(Log-rank=10.121、Log-rank=34.896、Log-rank=34.923、Log-rank=4.082,P均<0.05)。结论ESCC中CHI3L1高表达与Wnt/β-catenin信号通路密切相关,推测CHI3L1可能通过Wnt/β-catenin信号通路的异常激活参与ESCC的发生、发展。Purpose To investigate the expression of chitinase-3-like protein 1(CHI3L1)in esophageal squamous cell carcinoma(ESCC)and its correlations between CHI3L1 and molecules associated with the Wnt/β-catenin signaling pathway.Methods Paraffin-embedded tissue specimens from 119 ESCC patients and 60 normal esophageal tissues were retrospectively selected.Immunohistochemistry was used to detect the protein expression levels of CHI3L1 and Wnt/β-catenin signaling pathway related molecules in the above tissues,and the ectopic protein expression levels of CHI3L1,TCF4,Cyclin D1,APC andβ-catenin in ESCC patients’tissues and normal esophageal tissues were compared.The survival of ESCC patients was followed up.Results Compared with normal esophageal tissue,the positive rates of CHI3L1,TCF4,Cyclin D1 and β-catenin in ESCC were higher(63.0%,69.7%,68.1% and 67.2%,respectively),and the positive rates of APC were lower(41.2%).The expression levels of CHI3L1,TCF4,Cyclin D1 andβ-catenin were positively correlated with each other(P<0.05),but negatively correlated with APC(P<0.05).CHI3L1 protein was associated with ESCC tumor volume,invasion depth,lymph node metastasis,and pTNM stage(P<0.05).High expression of CHI3L1,TCF4,Cyclin D1 or abnormal high expression ofβ-catenin protein was associated with poor prognosis in ESCC patients,suggesting shortened survival time(Log-rank=10.121,Log-rank=34.896,Log-rank=34.923,Log-rank=4.082,all P<0.05).Conclusion In ESCC,the high expression of CHI3L1 is closely related to the activation of Wnt/β-catenin signaling pathway,suggesting that CHI3L1 may participate in the occurrence and development of ESCC through abnormal activation of Wnt/β-catenin signaling pathway.
