依达拉奉注射液联合静脉溶栓对急性脑梗死患者神经功能损伤、出血性转化及脑血流动力学的影响

摘　　要：目的:探究依达拉奉注射液联合静脉溶栓(IVT)对急性脑梗死(AIS)患者神经功能损伤、出血性转化及脑血流动力学的影响。方法:选取2021年1月—2023年12月鹰潭市人民医院急诊科收治的80例AIS患者,依据随机数字表法分为对照组(IVT治疗)与联合组(IVT联合依达拉奉治疗),各40例。比较两组神经功能损伤指标、出血性转化发生率及脑血流动力学。结果:联合组总有效率为97.50%,高于对照组的80.00%,差异有统计学意义(P<0.05)。治疗后,与对照组相比,联合组美国国立卫生研究院卒中量表(NIHSS)评分较低,差异有统计学意义(P<0.05)。治疗期间,联合组出血性转化(HT)发生率为10.00%,低于对照组的27.50%,差异有统计学意义(P<0.05)。治疗后,与对照组相比,联合组颈总动脉平均血流量(Qmean)、平均血流速度(Vmean)均较高,脑血管外周阻力(RV)、特性阻抗(ZCV)、动态阻力(DR)均较低,差异均有统计学意义(P<0.05)。结论:在常规治疗基础上,依达拉奉注射液联合IVT治疗AIS患者能提高临床效果,促进神经功能损伤修复,并能降低出血性转化,改善脑血流动力学。Objective:To explore the influence of Edaravone Injection combined with intravenous thrombolysis(IVT)on neurological impairment,hemorrhagic transformation and cerebral hemodynamics in patients with acute ischemic stroke(AIS).Method:A total of 80 AIS patients admitted to the Emergency Department of Yingtan City People's Hospital from January 2021 to December 2023 were selected and divided into control group(IVT treatment)and combined group(IVT combined with Edaravone treatment)according to random number table method,with 40 cases in each group.The indexes of nerve function injury,incidence of hemorrhagic transformation and cerebral hemodynamics were compared between the two groups.Result:The total effective rate of the combined group was 97.50%,which was higher than 80.00%of the control group,the difference was statistically significant(P<0.05).After treatment,the national institutes of health stroke scale(NIHSS)score of the combined group was lower than that of the control group,the difference was statistically significant(P<0.05).During treatment,the incidence of hemorrhagic transformation(HT)in the combined group was 10.00%,lower than 27.50%in the control group,the difference was statistically significant(P<0.05).After treatment,compared with control group,the average blood flow(Qmean)and average blood(Vmean)flow velocity of common carotid artery in combined group were higher,and the peripheral cerebral vascular resistance(RV),characteristic impedance(ZCV)and dynamic resistance(DR)were lower,with statistical significance(P<0.05).Conclusion:On the basis of routine treatment,Edaravone Injection combined with IVT can enhance the clinical efficacy,promote the repair of neurological impairment,reduce the hemorrhagic transformation and improve the cerebral hemodynamics in the treatment of AIS patients.
