血清GFAP、MIP-1α水平评估脑梗死后继发癫痫的价值

摘　　要：目的探讨血清胶质纤维酸性蛋白(GFAP)、巨噬细胞炎症蛋白-1α(MIP-1α)水平评估脑梗死后继发癫痫的临床价值。方法回顾性抽取2022年1月至2023年12月南阳市中心医院收治的19例脑梗死后继发癫痫患者作为观察组,另抽取63例脑梗死患者作为对照组。采用酶联免疫反应检测所有患者入院时的血清GFAP、MIP-1α水平。采用受试者工作特性曲线评估血清GFAP、MIP-1α水平对脑梗死后继发癫痫的临床价值。结果观察组血清GFAP、MIP-1α水平均高于对照组(P均<0.05)。血清GFAP、MIP-1α评估脑梗死后继发癫痫的曲线下面积(AUC)的95%CI分别为0.769(0.718~0.819)、0.801(0.756~0.847),两者联合预测的AUC(95%CI)为0.912(0.868~0.962);血清GFAP、MIP-1α的截点值是74.11 ng/L、30.17 pg/mL。结论脑梗死后继发癫痫患者血清GFAP、MIP-1α水平均呈高表达,可作为评估脑梗死患者继发癫痫的生物学指标,两者联合检测可提高评估效能。ObjectiveTo investigate the clinical value of serum levels of glial fibrillary acidic protein(GFAP)and macrophage inflammatory protein-1α(MIP-1α)in prediction on epilepsy secondary to cerebral infarction.MethodsA total of 19 patients with epilepsy secondary to cerebral infarction admitted to Nanyang Central Hospital from January 2022 to December 2023 were retrospectively selected as the observation group,and another 63 patients with cerebral infarction were selected as the control group.The serum levels of GFAP and MIP-1αwere detected by enzyme-linked immunosorbent assay.The clinical value of serum levels of GFAP and MIP-1αfor epilepsy secondary to cerebral infarction was investigated by receiver operating characteristic curve.ResultsThe serum levels of GFAP and MIP-1αin the observation group were higher compared with the control group(all P<0.05).The 95%confidence interval(95%CI)of area under the curve(AUC)of serum GFAP and MIP-1αfor epilepsy secondary to cerebral infarction were 0.769(0.718-0.819)and 0.801(0.756-0.847),respectively,and the AUC(95%CI)of combination of them was 0.912(0.868-0.962).The cut-off values of serum GFAP,MIP-1αwere 74.11 ng/L,30.17 pg/mL.ConclusionsSerum levels of GFAP and MIP-1αare highly expressed in patients with epilepsy secondary to cerebral infarction,which can be used as biological indicators to evaluate secondary epilepsy in patients with cerebral infarction.
