能谱CT在中轴型脊柱关节炎骶髂关节炎诊断中的应用价值

摘　　要：目的分析能谱CT对中轴型脊柱关节炎骶髂关节炎的诊断价值。方法选取2020年1月至2024年8月期间厦门大学附属中山医院收治的58例中轴型脊柱关节炎患者作为观察组,选取同期于本院接受健康体检人员29名作为对照组,所有研究对象均接受能谱CT扫描,分析检查结果。结果两组骶骨皮质下骨CT值以及骶侧水含量比值无统计学差异(P>0.05),观察组髂侧和骶侧钙含量比值、髂侧水含量比值均高于对照组(P<0.05),髂骨皮质下骨CT值低于对照组(P<0.05);58例患者CT分期中8例(13.79%)为0期,20例(34.48%)为Ⅰ期,8例(13.79%)为Ⅱ期,10例(17.24%)为Ⅲ期,12例(20.69%)为Ⅳ期,男女CT分期无明显差异(P>0.05)。结论中轴型脊柱关节炎骶髂关节炎患者在临床诊断中应用能谱CT,可以观察到患者骨损伤、硬化、水肿,实现定量分析,具有较高的临床应用价值。Objective To analyze the diagnostic value of spectral CT in sacroiliitis in patients with axial spondyloarthritis.Methods 58 patients with axial spondyloarthritis admitted to Zhongshan Hospital affiliated with Xiamen University from January 2020 to August 2024 were selected as the observation group,and 29 healthy individuals who underwent physical examinations at our hospital during the same period were selected as the control group.All study subjects underwent spectral CT scans,and the examination results were analyzed.Results The CT values of the sacral cortical bone and the sacral side water content ratio were similar between the two groups(P>0.05).However,the observation group exhibited significantly higher calcium and water content ratios on the iliac and sacral sides compared to the control group(P<0.05).Additionally,the sacral cortical bone CT value in the observation group was significantly lower than that in the control group(P<0.05).According to CT staging,8 patients(13.79%)were in stage 0,20 patients(34.48%)in stage I,8 patients(13.79%)in stage II,10 patients(17.24%)in stage III,and 12 patients(20.69%)in stage IV.No significant differences were observed between male and female patients in terms of CT staging(P>0.05).Conclusion Spectral CT can effectively observe bone damage,sclerosis,and edema in patients with sacroiliitis associated with axial spondyloarthritis,enabling quantitative analysis.This technique has significant clinical value for diagnosis.
