MRI联合AFP-L3预测肝癌射频消融术后复发的研究

摘　　要：目的 本研究探讨血清凝集素反应性甲胎蛋白(serum lectin-reactive alpha-fetoprotein,AFP-L3)结合M RI预测经皮射频消融(radiofrequency ablation,RFA)后肝癌复发的价值。方法 纳入94例肝癌RFA患者和82名健康人。磁共振成像记录表观弥散系数(apparent diffusion Coefficient, ADC),定量测定所有受试者血清AFP-L3的浓度,分析其与治疗后临床疗效的相关性和预测作用。结果 肝癌患者血清AFP-L3水平高于健康对照组,ADC值低于健康对照组。肿瘤残留者的ADC值低于完全消融者,血清AFP-L3水平高于完全消融者,联合检测ADC值和血清AFP-L3水平诊断完全消融的敏感性和特异度分别为87.50%和87.18%,肿瘤结节数、肿瘤直径、AFP、AFP-L3和有无肝硬变均为肝癌术后1年内复发的独立危险因素。ADC值与血清AFP-L3水平联合检测对预测肝癌复发的敏感性为92.86%,特异性为69.62%。结论 ADC值与血清AFP-L3检测相结合,对射频治疗后肝癌的完全消融和复发有较好的预测作用。Objective To investigate the value of serum ectin-reactive alpha-fetoprotein(AFP-L3)combined with MRI in predicting liver ca ncer recu rrence after percutaneous Radiofrequency a blation(RFA).Methods94 patients with RFA and 82 healthy subjects were included.The apparent diffusion Coefficient(ADC)was recorded by magnetic resonance imaging,and the concentration of AFP-L3 in serum of all subjects wa s qua ntitatively measured,and the co rrelation a nd predictive effect of AFP-L3 we re analyzed.Results The level of AFP-L3 in liver cancer patients was higher than that in healt hy control group,and the ADC value was lower than that in healthy control group.The ADC value of residual tumor was lower than that of complete ablation,and the serum AFP-L3 level was higher than that of com plete ablation.The sensitivity and specificity of combined detection of ADC value and serum AFP-L3 level in the diagnosis of complete ablation were 87.50%and 87.18%,respectively.The number of tumor nodules,tumor diameter,AFP,AFP-L3 and the presence or absence of cirrhosis were all independent risk factors for recurrence of HCC within 1 year after surgery.The sensitivity and specificity of ADC and AFP-L3 in predicting HCC recurrence were 92.86%and 69.62%respectively.Conclusion ADC value combined with serum AFP-L3 detection has a good predictive effect on complete ablation and recurrence of HCC after radiofrequency therapy.
