摘　　要：目的 分析糖链抗原19-9(CA19-9)、癌胚抗原(Carcinoembryonic antigen,CEA)、甲胎蛋白(AFP)检测联合128层多层螺旋(CT(MSCT)对早期食管癌的诊断效果。方法 收集本院2021年6月至2023年10月收治的92例食管癌患者的临床资料(食管癌组)。另同期选取在本院进行健康体检的63例健康体检者作为对照组。观察食管癌大小、形态、密度等CT征象,比较不同人群CA19-9、CEA、AFP水平;并绘制受试者工作特征曲线(ROC),分析MSCT联合CA19-9、CEA、AFP对早期食管癌的诊断价值。结果 食管癌组CA19-9、CEA、AFP水平均明显高于对照组(P<0.05)。食管癌Ⅰ-Ⅱ期患者血清CA19-9、CEA、AFP1水平明显低于Ⅲ-Ⅳ期者(P<0.05)。ROC曲线示,MSCT、CA19-9、CEA、AFP四者联合检测曲线下面积(AUC)、敏感度、特异度分别为0.915、0.965、0.863,均高于各项指标单独检测。结论 CA19-9、CEA、AFP在食管癌中均呈高表达,可能与食管癌发生、发展有关;且上述因子联合MSCT检查可提高早期食管癌诊断价值。Objective To analyze the diagnostic effect of Carbohydrate antigen 19-9(CA19-9),Carcinoembryonic antigen(CEA),Alpha fetoprotein(AFP)detection combined with 128-slice multi-slice spiral CT(MSCT)in the diagnosis of early esophageal cancer.Methods The clinical data of 92 patients with esophageal cancer admitted to our hospital from June 2021 to October 2023 were collected(esophageal cancer group).63 healthy people who underwent physical examination during the same period in our hospital were selected as the control group.Observed the CT signs of esophageal cancer such as size,shape and density,compared the levels of CA19-9,CEA,and AFP in different populations;drew a receiver operating characteristic curve(ROC),and analyzed the diagnostic value of the combination of MSCT and CA19-9,CEA,AFP for early esophageal cancer.Results The levels of CA19-9,CEA and AFP in the esophageal cancer group were significantly higher than those in the control group(P<0.05).Serum levels of CA19-9,CEA and AFP1 in patients with stageⅠ-Ⅱesophageal cancer were significantly lower than those with stageⅢ-Ⅳ(P<0.05).The ROC curve showed that the area under the curve(AUC),sensitivity,and specificity of the combined detection of MSCT,CA19-9,CEA,and AFP were 0.915,0.965,and 0.863,respectively,which were all higher than those detected separately.Conclusion CA19-9,CEA,and AFP are all highly expressed in esophageal cancer,which may be related to the occurrence and development of esophageal cancer;and the combination of the above factors and MSCT can improve the diagnostic value of early esophageal cancer.
