摘 要:目的:探讨MRI扫描技术对于食管鳞状细胞癌患者放疗靶区及危及器官的影响,为临床治疗提供参考。方法:选取医院收治的食管癌患者,分别行CT及MRI扫描,并根据CT及MRI、CT融合图像勾画放疗靶区,设计放疗计划。对两组计划的靶区大小及心脏、肺的剂量学参数进行比较。结果:MRI、CT融合组患者靶区平均体积少于CT组(P<0.05);接受20Gy照射的肺实质体积(V20)少于CT组(P<0.05)。两组患者心脏平均剂量(cGy)之间差异无统计学意义(P=0.085);同时食管癌发病部位与两组的靶区体积的差异无统计学意义(H=2.02,P=0.365);与心脏平均剂量(cGy)的差异无统计学意义(H=2.01,P=0.367),与肺V20之间差异无统计学意义(H=1.01,P=0.604)。结论:在食管鳞癌患者中,相对于CT图像,MRI、CT图像融合对心脏器官危害影响虽然相似,但对于肺部器官危害更小,且能够更准确识别肿瘤大小,缩小靶区体积,同时与食管癌发病部位无关。Objective:To investigate the impact of MRI scanning technology on radiotherapy target volume and organs at risk in patients with esophageal squamous cell carcinoma,and to provide reference for clinical treatment.Methods:Patients with esophageal cancer admitted to hospital were selected for CT and MRI scan.Radiotherapy target volume were delineated according to CT or MRI、CT fusion images,and radiotherapy plans were designed respectively.The target size and dosimetric parameters of heart and lung were compared between the two planned groups.Results:The mean target volume(cm^(3))of the MRI-CT fusion group was less than that of the CT group(P<0.05);the volume of lung tissue receiving 20 Gy irradiation(V20)(%)was less than that of the CT group(P<0.05).There was no significant difference in the mean heart dose(cGy)between the two groups(P=0.085);the difference in target volume between the two groups was not statistically significant(H=2.02,P=0.365);the difference in mean heart dose(cGy)was not statistically significant(H=2.01,P=0.367),and the difference in lung V20(%)not statistically significant(H=1.01,P=0.604).Conclusion:In patients with esophageal squamous cell carcinoma,the impact of MRI and CT fusion groups on cardiac organ toxicity was similar to that of the group,but it was less harmful to the lungs,and could more accurately identify the size of the tumor,reduce the target volume,and was not related to location of the esophageal cancer.
