经阴道彩色多普勒超声与阴道镜检查在宫颈癌诊断中的应用价值

摘　　要：目的:比较经阴道彩色多普勒超声与阴道镜检查在宫颈癌诊断中的效能。方法:选取2020年11月—2023年11月于广州市白云区妇幼保健院就诊的疑似宫颈癌患者50例为研究对象,均给予经阴道彩色多普勒超声与阴道镜检测,以宫颈活检结果为“金标准”,比较不同检测方法的准确度、灵敏度、特异度、阳性预测值、阴性预测值。结果:宫颈活检诊断宫颈癌阳性32例,阴性18例;经阴道彩色多普勒超声检测阳性28例,阴性22例;阴道镜检测阳性30例,阴性20例。阴道镜诊断宫颈癌准确度高于经阴道彩色多普勒超声(P=0.046);经阴道彩色多普勒超声与阴道镜诊断宫颈癌灵敏度、特异度、阴性预测值、阳性预测值比较,无统计学差异(P>0.05)。结论:阴道镜诊断宫颈癌的准确度高于经阴道彩色多普勒超声,但两者均可作为有效的辅助工具,对宫颈癌早期筛查具有重要意义。Objective:To compare the efficacy of transvaginal color Doppler ultrasonography and colposcopy in the diagnosis of cervical cancer.Methods:Fifty patients with suspected cervical cancer who were treated in Guangzhou Baiyun District Maternal and Child Health Hospital from November 2020 to November 2023 were selected as the study objects,and all of them were tested by transvaginal color Doppler ultrasonography and colposcopy.With cervical biopsy results as the"gold standard",the accuracy,sensitivity,specificity,positive predictive value and negative predictive value of different detection methods were compared.Results:Cervical biopsy diagnosed 32 positive cases of cervical cancer,18 negative cases;transvaginal color Doppler ultrasonography was positive in 28 cases and negative in 22 cases;colposcopy tests were positive in 30 cases and negative in 20 cases;the diagnostic accuracy of colposcopy was higher than that of transvaginal color Doppler ultrasonography(P=0.046);there was no significant difference in sensitivity,specificity,negative predictive value and positive predictive value between transvaginal color Doppler ultrasonography and colposcopy(P>0.05).Conclusion:The accuracy of colposcopy in the diagnosis of cervical cancer is higher than that of transvaginal color Doppler ultrasonography,it is of great significance for early screening of cervical cancer.
