摘　　要：目的　评价双侧子宫动脉栓塞术治疗子宫肌瘤的临床疗效。方法　采用 Seldinger方法作选择性子宫动脉栓塞治疗 2 2例子宫肌瘤患者 ,栓塞材料选用 PVA颗粒 (直径 35 0～ 5 0 0 μm) ,真丝微粒 ( 4 #真丝线剪成 1 mm长 )及线段 ( 4 #真丝线剪成 1 0 mm长 ) ,以完全阻断子宫动脉血流为宜。结果　血管造影发现 ,子宫肌瘤血供丰富 ,均由两侧子宫动脉供血 ,栓塞后肌瘤的血供可阻断。随访 3～ 6个月结果表明 :月经量明显减少 ,经期有不同程度缩短 ,3例闭经 ,其中 1例又复潮。术后 3个月子宫肌瘤平均缩小 46.7% ,其中缩小80 % 3例 ,5 0 %～ 70 % 6例 ,2 5 %～ 40 % 6例 ,<2 0 % 3例。术后 6个月有 3例原已缩小的肌瘤复又增大。结论　UAE创伤小 ,近期疗效明显 ,而远期疗效尚需观察 。Objective To evaluate the clinical effects of uterine arterial embolization(UAE)in the treatment of uterine fibroid(UF),Methods 22 cases with symptomatic uterine fibroid were treated by Seldinger's selective uterine arteries embolization with PVA particles of 350~500μ m and 4# silk fragments in order to occlude the blood supply of fibroid.Results Arteriography showed that most unterine myomata were supplied bilateral unterine arteries.when both unterine arteries were embolized,the blood supply of uterine myoma could be occluded With a mean follow up of 3~6 months the menophania and menstrual cycles eturned to normal.The volume of UF decreased to 46 7% at 3 months 3 cases of decreased returned to increased at 6 months.Conclusions UAE is a effective method with less trauma,longer follow up is needed to evaluate the long term effects.
