60例上皮源性鼻腔癌的临床疗效分析

摘　　要：目的　评价上皮源性鼻腔癌的临床疗效及影响预后的因素。方法　回顾性分析 1985～1992年收治的 6 0例上皮源性鼻腔癌。其中单纯放射治疗 34例 ,手术 +放射治疗 2 6例。生存统计采用Kaplan Meier法 ,组间比较采用Log rank检验 ,多因素分析采用Cox模型。结果　总 5年生存率为5 5 .9% ,10年生存率为 36 .9%。早期患者 (Ⅰ、Ⅱ期 )的 5 ,10年生存率分别为 79.0 %和 5 7.9% ,晚期患者 (Ⅲ、Ⅳ期 )的 5 ,10年生存率则分别为 4 4 .1%和 2 6 .0 % ,二者差异有显著性 (P =0 .0 0 5 )。手术 +放射治疗与单纯放疗的生存率差异无显著性 (P =0 .33) ,鳞癌的生存率明显低于腺癌 (P =0 .0 4 ) ,初诊时颈部有淋巴结转移者的 5 ,10年生存率低于无颈部淋巴结转移者 (P =0 .0 9)。结论　治疗方式、颈部淋巴结转移对预后无明显影响 ,而病理类型、临床分期对预后有显著影响。Objective To evaluate the treatment modalities and the prognostic factors of nasal cavity carcinoma. Methods A retrospective study was done on 60 nasal cavity carcinoma patients treated from 1985 to 1992. Thirty-four patients received radiotherapy alone and 26 patients received surgery plus radiotherapy. Kaplan-Meier method was used to evaluate the survival, Log-rank test to assess the difference between these two groups and Cox proportional hazard model by multivariate analysis. Results The overall 5- and 10-year survival rates were 55.9% and 36.9% respectively. The 5- and 10-year survival rates were 79.0% and 57.9% in patients with early lesions (stage Ⅰ,Ⅱ) and 44.1% and 26.0% in patients with advanced lesions (stage Ⅲ,Ⅳ)(P= 0.005). There was no significant difference in the survival rate between radiotherapy alone and radiotherapy plus surgery (P=0.33). Patients with squamous cell carcinoma had obviously poorer survival rate than patients with adenocarcinoma (P=0.04). Patients with positive nodes had a lower survival rate compared with negative node patients (P=0.09). Conclusion Histological type and clinical stage, but not method of treatment or neck node metastasis, are the important prognostic factors.
