摘  要:目的 研究肺动脉是否参与支气管肺癌的供血及其在肺癌中的形态学改变。方法 对 2 0例支气管肺癌患者(中央型 14例 ,周围型 6例 )分别行肺动脉CTA和DSA检查。结果  2 0例中 ,12例DSA显示肺癌所在区域内 ,肺动脉血流以段和叶为单位灌注不足或无血流灌注 ,其CTA显示肺动脉分支被肿块包埋、管腔缩小或者未见肺动脉分支显影。 8例肺动脉DSA显示肺癌所在区域内肺动脉管径及肺血灌注正常 ,其对应的CTA显示瘤肺交界区的肺动脉分支与肺癌相贴、管径基本正常。 2 0例肺动脉DSA及CTA均未见肺动脉形成肿瘤血管。结论 肺动脉不参与支气管肺癌供血 ,肺癌的经导管动脉化疗和栓塞不应经肺动脉途径。Purpose To investigate whether or not the pulmonary artery participates in the blood supply of lung cancer and its change of morphology in lung cancer. Methods The DSA and CTA of pulmonary artery were performed simultaneously in 20 patients with bronchogenie carcinoma (14 center tumors and 6 peripheral tumors). Results DSA of 12 patients showed that pulmonary blood flow and perfusion were reduced or absent in the affected area and CTPA(CTA of pulmonary artery ) showed that the branch of pulmonary artery were narrowed or encased by the tumor, and some of them were not able to be seen. DSA of 8 patients showed the branches of pulmonary artery and pulmonary blood perfusion were normal in the affected area and CTPA showed that the branches of pulmonary artery were normal or attached to the tumor. DSA and CTA of pulmonary artery of 20 patients disclosed that the pulmonary artery did not form tumor vessel. Conclusion The pulmonary artery doesn't participate in the blood supply of lung cancer.It is unreasonable to perform transcatheter chemo embolization for lung cancer via pulmonary artery.
