摘 要:目的 提高对局限性放射性肝损伤的CT影像认识。方法 以 13例肝癌经三维适形放射治疗 ( 3 dimensionalconformalradiationtherapy ,3 DCRT) ,且经临床确诊为并发局限性放射性肝损伤病例的放疗前后CT影像进行回顾性对比分析。结果 13例肝癌病例在放疗结束后 1~ 2个月CT复查时 ,均在肝肿瘤区 (grosstumorvolume ,GTV)周围出现宽窄不一、形状各异的低密度带。结论 结合 3 DCRT放疗病史 ,对放疗前后CT影像和临床、化验检查资料进行综合分析对比 ,可以对局限性放射性肝损伤作出正确诊断。局限性放射性肝损伤的发生和分布范围与 3 DCRT治疗计划的放射剂量分布有关 ;与肝脏的解剖结构无关。Objective To realize the CT appearances of local radiation induced hepatic injury. Methods CT scans of 13 patients with primary hepatic malignant tumor were retrospectively analyzed before and after three dimensional conformal radiation therapy (3 DCRT), which had clinical evidence of radiation injury. Results The follow up CT studies of 13 patients after completion of therapy 1-2 months showed a low attenuation area adjacent gross tumor volume(GTV). Conclusion The local radiation induced hepatic injury is evidently diagnosed on undergoing 3 DCRT, CT scan before and after therapy and abnormal results of liver function tests. The occurrence and distribution of local radiation induced hepatic injury are correspond to the isodose curve in the three dimensional treatment planning, not relative to the anatomy of liver.
