摘　　要：目的　探讨超声引导经皮穿刺微波治疗骨肿瘤的可行性和疗效。方法　 12例骨肿瘤患者 ,15个肿块 ,大小 2～ 6cm。骨肉瘤 5例 ,转移性骨肿瘤 7例 ,因各种原因不能手术。超声引导下经皮穿刺骨肿瘤导入微波治疗并测温 ,功率 3 0～ 40W ,时间 3 0 0～ 60 0s。结果　治疗后局部疼痛缓解或消失 ,肢体活动改善 ,血钙及血清碱性磷酸酶下降。Enneking功能评价 15～ 2 0分 8例 ,大于 2 1分 4例 ,平均 2 3分。彩色多普勒血流显像肿瘤均缩小 ,境界清晰 ,内部回声增强 ,血流减少或消失。CT检查 12例 15个肿块缩小 ,11个无强化 ,4个周边少许强化。随访 3～ 3 6个月 ,8例存活。无严重并发症发生。结论　超声引导经皮穿刺微波治疗骨肿瘤安全、有效、可行 ,并发症少 ,克服了外科和理疗微波治疗的不足 ,是一种治疗骨肿瘤较好方法。Objective To evaluate the feasibility and therapeutical effect of ultrasound guided percutaneous microwave coagulation therapy for malignant bone tumor. Methods The ultrasound microwave therapy 1(UMC 1)equipment was utilized to treat 12 patients with malignant bone tumor, with an output of 30-40W lasting 300-600s. In these cases, osterosarcoma in 5 and bone metastastic tumors in 7. Results Eight patients remained alive after 3-36 months. After the treatment, all lessions were decreased in size and blood flow in nodules disappeared. There were no enhancement in 11 tumors by enhanced CT scans. The level of ALP and calcium in the blood decreased, patients felt the pain disappeared or milder than before. The scores of the functional evaluation by Enneking system were≥21 in 4, 15-20 in 8, without serious complication. Conclusion Ultrasound guided microwave treatment is a safe, effective, feasible method for malignant bone tumors, it is better than microwave treatment during the surgical procedure.
