CD44^+/CD24^-和CK5/6在乳腺浸润性导管癌、乳腺普通型导管增生和低级别导管原位癌中的表达

摘　　要：目的探讨乳腺浸润性导管癌、普通型导管增生和低级别导管原位癌组织中CD44^+/CD24^-和CK5/6的表达和意义。方法采用免疫组织化学染色检测30例乳腺浸润性导管癌中CD44^+/CD24^-、CK5/6和P63的表达、30例乳腺普通型导管增生组织中CD44^+/CD24^-、CK5/6、34βE12和CK8的表达和30例低级别导管原位癌中CD44^+/CD24^-、CK5/6的表达。结果 30例乳腺浸润性导管癌中24例呈CD44^+/CD24^-,30例普通型导管增生中9例呈CD44^+/CD24^-和30例低级别导管原位癌中19例呈CD44^+/CD24^-,3组相比具有统计学意义。浸润性导管癌中CD44^+/CD24^-表型与患者的年龄、月经状况、肿瘤大小、淋巴结转移、组织学分级、ER、PR和HER^-2无相关性。30例乳腺浸润性导管癌中6例表达CK5/6,30例乳腺普通型导管增生中全部表达CK5/6,30例低级别导管原位癌中8例表达CK5/6,3组相比具有统计学意义。结论 CD44^+/CD24^-和CK5/6联合使用可用于鉴别乳腺浸润性导管癌、普通型导管增生和低级别导管原位癌。Objectives To explore the expression and significance of CD44^+/ CD24-and CK5 /6 in invasive ductal breastc arcinoma,usual ductal hyperplasia and low grade ductal carcinoma in situ. Methods Double immunohistochemical staining of CD44 / CD24 was performed in 30 cases of invasive ductal carcinoma,30 cases of usual ductal hyperplasia and 30 cases of low grade ductal carcinoma in situ. Meanwhile,single immunohistochemical staining of CK5 /6 and P63 was performed in invasive ductal carcinoma,single immunohistochemical staining of CK5 /6,34βE12 and CK8 in usual ductal hyperplasia,and single immunohistochemical staining of CK5 /6 in low grade ductal carcinoma in situ. Results 24,9,and 19 cases of CD44^+/ CD24-phenotype were detected in invasive ductal carcinoma,usual ductal hyperplasia and low grade ductal carcinoma in situ,respectively. There were significant differences among the 3 groups.No significant correlation was found between the CD44^+/ CD24-phenotype and patients’ age,menstruation state,tumour size,lymph node metastasis,histological grade,ER,PR and HER- 2. 6 cases of invasive ductal carcinoma,totally 30 cases of usual ductal hyperplasia and 8 cases of low grade ductal carcinoma in situ were CK5 /6 positive. Significant differences were observed among the 3 groups.Conclusion CD44^+/ CD24-and CK5 /6 can be used in combination to diagnose invasive ductal carcinoma,usual ductal hyperplasia and low grade ductal carcinoma in situ.
