摘 要:目的检测浸润性导管癌(infiltrating ductal carcinoma,IDC)中上皮间充质转化(epithelial-mesenchymal transition,EMT)相关标记物Twist蛋白和maspin蛋白的表达,并分析它们之间的相互关系以及临床病理意义。方法收集临床病例325例石蜡包埋IDC、50例导管原位癌以及50例正常乳腺组织,应用免疫组织化学ElivisionTMplus法检测IDC、原位癌和正常乳腺组织中Twist和maspin蛋白的表达。结果 Twist在IDC组、原位癌组、正常对照组中的阳性表达率为55.7%、24.0%、2.0%,表达逐渐降低,差异有统计学意义;maspin蛋白在IDC组、原位癌组和正常对照组中的阳性率分别为40.9%、80.0%、100%,表达逐渐升高,差异有统计学意义。Twist蛋白的表达与IDC组织的淋巴结转移与否及临床分期及术后生存期均有关;随着maspin蛋白的阳性表达率的降低,IDC组织的分化越差、淋巴结易发生转移、临床分期越高,差异有统计学意义,且maspin蛋白阳性表达组患者的生存时间明显高于其阴性表达组患者。Spearman相关分析显示在IDC中,Twist蛋白和maspin蛋白呈负相关。Cox回归分析显示Twist和maspin蛋白的阳性表达以及TNM分期是影响IBC患者术后生存的独立预后因素。结论Twist和maspin蛋白的异常表达可能参与了乳腺癌的发生、发展及转移过程;Twist和maspin可以作为预测IDC患者浸润、转移及预后的标记物。Objective To investigate the expression of the EMT( epithelial-mesenchymal transition) related marker of Twist and the tumor suppressor of maspin in the primary infiltrating ductal carcinoma( IDC), and their correlation as well as their clinicopathological significance. Methods We collected clinical cases including 325 DIC, 50 carcinoma in situ and 50 normal mammary tissues specimens. The expression of Twist and maspin proteins was examined by immunohistochemistry ElivisionTMplus in, carcinoma in situ, and normal mammary tissues. Results The positive rates of expression of Twist and maspin were 2. 0% and 100%, respectively, in normal mammary tissues; 24. 0% and 80. 0%, respectively, in CIS tissues; and 55. 7% and 40. 9%, respectively, in IDC tissues. There were significant differences among of the three groups( P < 0. 05). The expression of Twist protein was significantly related to lymph node metastasis, TNM stages, and postoperative survival time. There was a negative correlation between the expression of maspin protein and lymph node metastasis and TNM stages, and the survival time of the maspin-positive group was significantly longer than that of the negative group. Spearman analysis showed that there was a negative relationship between Twist expression and maspin expression( r=- 0. 190, P < 0. 01). Cox regression analysis showed that the expression of Twist and that of maspin and TNM stages were independent prognostic factors of IDC. Conclusions The abnormal expression of Twist and maspin may be involved in the initiation, development, and metastasis of IDC. Twist and maspin may be considered markers of predicting invasion, metastasis, and prognosis of IDC.
