摘　　要：目的　探讨血管内皮生长因子在妇科肿瘤中的表达及其判断预后的价值。方法　采用ELISA方法检测 13 1例妇科肿瘤患者及 92例健康献血员血清VEGF(S -VEGF)浓度并分析S -VEGF浓度在治疗中的变化及意义。结果　常见妇科恶性肿瘤治疗前S -VEGF浓度明显高于健康对照组及妇科良性肿瘤组的S -VEGF浓度 (P <0 .0 1) ;中晚期妇科恶性肿瘤患者、术中有残留者、组织分化差者、治疗后半年内有复发者其S -VEGF浓度分别较早中期者、术中无残留者、组织分化好者、治疗后半年内无复发者明显升高 (P <0 .0 1) ;手术后 1周内S -VEGF浓度较术前明显升高 (P <0 .0 1)。结论　妇科恶性肿瘤患者S -VEGF浓度明显高于健康对照组及妇科良性肿瘤组 ;临床分期、病理分级、术中有残留及手术创伤皆显著影响S -VEGF值的表达。用多变量分析显示 ,S -VEGF浓度可作为影响无瘤生存的独立预后指标。Objective To analyse the expression and the significance of serum vascular endothelial growth factor(S VEGF) in patients with gynecological carcinoma.Methods The S VEGF concentrations of 131 patients with gynecological tumors and 92 healthy controls were measured by ELISA to analyse the alteration and significance in the treatment.Results The mean S VEGF level in patients with malignant gynecological tumor was significantly higher than in either patients with leiomyoma( P <0.01) or healthy controls( P <0.01).The mean S VEGF level in patients with advanced stage carcinoma,in patients with residual tumor,in patients with poorly differentiated carcinoma,in patients whose tumor relapsed within six monthes after treatment was significantly higher respectively than in patients with early stage carcinoma,in patients with nonresidual tumor,in patients with well differentiated carcinoma,in patients whose tumor did not relapse( P <0.01).The mean S VEGF level during one week of postoperation was significantly higher than that of preoperation( P <0.01).Conclusion The mean S VEGF level in patients with malignant gynecological tumor is significantly higher than in either patients with benign gynecological tumor or healthy controls.The clinical stage,the histological grade,the residual tumor,and the operative trauma all significantly influence the expression of S VEGF.Mutivariated analysis reveals that the S VEGF level is the independent prognostic target for cancer free survivorship.
