摘 要:目的 了解鼻咽癌放疗后耳聋的现状。方法 对 162例 ( 3 16耳 )鼻咽癌放疗后耳聋患者进行纯音听阈测试 ,并对测试的结果进行统计学分析。结果 神经性耳聋占 2 6.6% ( 84/ 3 16) ,传导性耳聋占 4.7% ( 15 / 3 16) ,混合性耳聋占 68.7% ( 2 17/3 16) ;95 .3 % ( 3 0 1/ 3 16)的耳有骨导听损 ,放疗后 10年内耳聋患者骨导听损较轻 ,放疗 10年后耳聋患者骨导听损明显加重 ;73 .4%( 2 3 2 / 3 16)的聋耳有气导听损 ,放疗后不同时间段传导性耳聋和混合性耳聋的平均骨气导差稳定在 2 7dB左右。结论 骨导听损虽然广泛 ,但程度较轻 ,发展缓慢 ;气导听损相对恒定 ,但程度较重。放疗后的耳聋主要是气导听损所致 ,通过治疗 ,有望得以控制。Objective To comprehend the status of deafness after radiotherapy in nasopharyngeal carcinoma(NPC).Methods The hearing thresholds were examinated in 162 cases(316 ears) of the deafness after radiotherapy in NPC by audiometer,and the results were analysed.Results There was sensorineural deafness in 26.6%(84/316) ears,the conductive deafness was 4.7%(15/316),the mixed deafness was 68.7%(217/316);there was hearing loss of 95.3%(301/316) ears in bone conduction(BC),hearing loss in BC showed lightly within decaded,but it increases markedly after 10 years;There was air conduction(AC) hearing loss in 73.4% (232/316) ears,the bone air conduction difference in conductive deafness and mixed deafness maintained at 27 dB.Conclusion The hearing loss in BC after radiotherapy are common,but its development is slowly.The main cause of deafness after radiotherapy is hearing loss in AC,it leads the worst effect for patient,the deafness would be controlled by treatment
